感染性心内膜炎患者个体化抗感染治疗的病例分析

摘　　要：合理、准确地使用抗菌药物是感染性心内膜炎治疗的关键。本文报道了1例感染性心内膜炎患者的抗感染治疗过程。患者外院治疗效果不佳、未明确感染病原体,在我院治疗过程中肌酸酐清除率处于较高水平。患者治疗期间,临床药师加入诊疗团队、参与制定个体化抗感染治疗方案。根据患者病原学结果、肾功能情况及抗菌药物药代动力学特点等加以分析,患者经过多次用药调整后,病情较前明显好转,感染指标及体温降至正常范围,无药物不良反应发生。患者一般情况好,感染得到了有效控制,转当地医院继续治疗。本文通过1例感染性心内膜炎患者的用药点评,旨在探讨此类患者抗感染药物的合理应用及临床药师开展药学服务工作的切入点。Objective Rational and accurate use of antibiotics is the key to the treatment of infective endocarditis. A case of infective endocarditis is reported in this paper. The treatment effect of the patient outside the hospitals was poor and the pathogen of infection was not clear. The creatinine clearance rate of patient was at a high level. During the treatment of patients, clinical pharmacists join the clinical team and participate in the formulation of individualized anti-infection treatment plans. Based on the analysis of etiological results, renal function, and pharmacokinetic characteristics of antibiotics, after several times of medication adjustment, the patient’s condition was significantly improved, infection indicators and body temperature came back to the normal range, and no adverse reactions occurred. The patient was general in good condition, the infection has been effectively controlled, and he was transferred to a local hospital for further treatment. The purpose of this study was to explore the rational use of anti-infective drugs in a patient with infective endocarditis and the entry point for clinical pharmacists to carry out pharmaceutical care.
