参松养心胶囊联合螺内酯胶囊治疗老年慢性心力衰竭临床研究

摘　　要：目的:观察参松养心胶囊联合螺内酯胶囊治疗老年慢性心力衰竭(CHF)的临床疗效及对患者运动耐力、炎症因子的影响。方法:选取老年CHF患者90例,按随机数字表法分为治疗组与对照组各45例。对照组在常规对症治疗的基础上加用螺内酯胶囊治疗,治疗组在对照组的基础上加用参松养心胶囊治疗。连续治疗2个月,比较2组临床疗效、中医证候积分、血清炎症因子[超敏C-反应蛋白(hs-CRP)、白细胞介素-6 (IL-6)、肿瘤坏死因子-α (TNF-α)]水平及运动耐力情况。结果:治疗组总有效率为95.56%,对照组为71.11%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组中医证候积分比较,差异无统计学意义(P>0.05);治疗后,2组中医证候积分均较治疗前明显下降(P<0.05),且治疗组中医证候积分低于对照组(P<0.05)。治疗前,2组血清hs-CRP、IL-6、TNF-α水平比较,差异无统计学意义(P>0.05);治疗后,2组血清hs-CRP、IL-6、TNF-α水平均较治疗前下降(P<0.05),且治疗组上述各项指标均低于对照组(P<0.05)。治疗前,2组6 min步行距离比较,差异无统计学意义(P>0.05);治疗后,2组6 min步行距离均较治疗前增加(P<0.05),且治疗组6 min步行距离大于对照组(P<0.05)。结论:参松养心胶囊联合螺内酯胶囊治疗老年CHF,能有效提高临床疗效,减轻炎症反应,提高患者运动耐力。Objective: To observe the clinical effect of Shensong Yangxin capsules combined with spironolactone capsules for chronic heart failure(CHF) in the senile and its effect on exercise endurance and inflammatory factors. Methods:A total of 90 cases of senile CHF patients were selected and divided into the treatment group and the control group according to the random number table method, with 45 cases in each group. The control group was additionally treated with spironolactone capsules based on routine symptomatic treatment, and the treatment group was additionally treated with Shensong Yangxin capsules based on the treatment of the control group. After two-month treatment,the clinical effects,the Chinese medicine syndrome scores, the levels of serum inflammatory factors including high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α),and the exercise endurance in the two groups were compared. Results:The total effective rate was 95.56% in the treatment group and 71.11% in the control group,the difference being significant(P<0.05). Before treatment,there was no significant difference being found in the comparison of Chinese medicine syndrome scores between the two groups(P>0.05). After treatment, the Chinese medicine syndrome scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the score in the treatment group was lower than that in the control group(P<0.05). Before treatment, there was no significant difference being found in the comparison of levels of hs-CRP,IL-6 and TNF-α in serum between the two groups(P>0.05).After treatment,the levels of hs-CRP,IL-6 and TNF-α in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the above levels in the treatment group were lower than those in the control group(P<0.05).
