摘  要:目的 通过可视化分析探索糖尿病相关认知功能障碍(diabetes mellitus-associated cognitive dysfunction,DMACD)发展脉络、研究热点和趋势,为后续研究提供参考。方法 采用CiteSpace5.7R1软件对中国知网自数据库建库至2020年8月31日收录的糖尿病相关认知功能障碍期刊论文和硕博士学位论文的作者、机构及关键词进行分析。结果 共1549篇文献纳入研究分析,结果显示(1)DMACD发展过程分为初时期(1991-2004年)、发展期(2005-2015年)和平稳期(2016-2019年)3个阶段,尚未形成核心研究团队和机构。(2)涉及机制包括胰岛素抵抗、细胞凋亡、氧化应激、Aβ沉积、tau蛋白过度磷酸化、脑源性神经影响因子、内质网应激、炎症等。(3)蒙特利尔认知评估量表(montreal cognitive assessment,MoCA)和静息态功能磁共振成像(resting-state functional magnetic resonance,rs-fMRI)是评估认知水平和脑功能的主要方式。(4)治疗药物以中医药的滋补脾阴方药,天然药物的小檗碱以及现代医学的利拉鲁肽为代表。其中,炎症机制的参与、rs-fMRI作为主要评估方式和利拉鲁肽的神经保护作用是近3年该领域研究重点。结论 运用可视化分析梳理文献,初步得到DMACD研究知识框架、热点和趋势。今后研究工作中需要结合目前研究方向和热点,加强各地区作者和机构之间的合作与交流,开展高质量的随机对照试验。Objective To explore the development context, research hotspots and trends of diabetes mellitus-associated cognitive dysfunction by visual analysis, and to provide the basis for the follow-up research,.Methods CiteSpace5.7 R1 software was used to analyze the authors, institutions and keywords of journal papers and master’s and doctoral dissertations on diabetes mellitus-associated cognitive dysfunction from the establishment of CNKI database to August 31, 2020.Results A total of 1549 literatures were included in the research and analysis. The results showed that(1) the research development process was divided into three stages: the initial stage(1991-2004), the development stage(2005-2015) and the stable stage(2016-2019). However, it had not yet formed a core research team and organization.(2) The mechanisms involved include insulin resistance, apoptosis, oxidative stress, Aβ, tau hyperphosphorylation, BDNF,endoplasmic reticulum stress, inflammation and so on.(3) MoCA and rs-fMRI were the main ways to evaluate cognitive function and brain function.(4) The therapeutic drugs were represented by nourishing spleen yin, berberine and liraglutide. The participation of inflammatory mechanism, rs-fMRI as the main evaluation method and the neuroprotective effect of liraglutide were the research focuses in this field in recent three years.Conclusion Combing the literature via visual analysis, the knowledge framework, hotspots and trends of DMACD research are preliminarily obtained. In the future, it is necessary to combine the current research direction and hot spots, strengthen the cooperation and exchange between authors and institutions in various regions, and carry out high-quality randomized controlled trials.
