磁圆梅针联合康复训练对缺血性中风后肢体痉挛患者神经及血管功能损伤的调节作用

摘　　要：目的比较磁圆梅针叩刺督脉结合康复训练与单纯磁圆梅针叩刺督脉、单纯康复训练治疗中风后肢体痉挛的临床疗效。方法将96例中风后肢体痉挛患者随机分为叩刺康复组、叩刺组和康复组,每组32例。叩刺康复组采用磁圆梅针叩刺督脉结合康复治疗,叩刺组采用叩刺治疗,康复组采用康复训练治疗,每周5次,4周为1个疗程,连续治疗2个疗程;分别对治疗前后改良的Ashworth评分(MAS)、Fugl-Meyer评分(FMA)、Barthel指数(BI)评分进行比较,同时对治疗前后血清中的轴突导向因子(Netrin-1),脑源性神经生长因子(Brain-Derived Neurotrophic Factor,BDNF),血管细胞黏附分子-1(Vascular cell adhesion protein 1,VCAM-1)以及血管内皮生长因子(Vascular Endothelial Growth Factors,VEGF)水平进行比较。结果三组患者治疗后MAS、FMA及BI评分较治疗前均有显著性差异(P<0.05),且叩刺康复治疗比叩刺治疗及康复治疗更能有效改善MAS、FMA及BI评分水平(P<0.05)。同时,叩刺康复治疗比叩刺治疗及康复治疗更能有效调节血清中的Netrin-1、BDNF、VCAM-1以及VEGF水平(P<0.05)。结论磁圆梅针叩刺督脉结合现代康复可以通过改善患者神经功能及缓解血管功能损伤,从而更好的改善中风后肢体痉挛的患肢肌张力,缓解患肢痉挛状态,提高运动功能和日常生活能力。Objective To compare the clinical effects of round magnetic needle tapping governor meridian combined with rehabilitation training, round magnetic needle tapping governor meridian and rehabilitation training for the treatment of ischemic post-stroke spasticity. Methods Totally 96 patients with ischemic post-stroke spasticity were randomly divided into tapping rehabilitation group, tapping group and rehabilitation group according to the time of admission, with 32 cases in each group. The percussion rehabilitation group used round magnetic needle combined with rehabilitation therapy. The percussion group used the same treatment as the percussion in the percussion rehabilitation group. The rehabilitation group adopted the same treatment as the rehabilitation technique in the percussion rehabilitation group, 5 times a week, 4 weeks as a course of treatment, 2 consecutive courses of treatment. The MAS,FMA, BI scores were observed before and after treatment, and the levels of serum Netrin-1, BDNF, VCAM-1, VEGF in these three groups were compared as well.Results After treatment, the scores of MAS、FMA and BI in the three groups were significantly different than those before treatment(P<0.05), and the changes of MAS, FMA, and BI scores after tapping plus rehabilitation treatment were significantly different than other two treatments(P< 0.05). Meanwhile, the changes of serum Netrin-1, BDNF, VCAM-1, VEGF level after tapping plus rehabilitation treatment were significantly different than two other treatments(P< 0.05).Conclusion Round magnetic needle tapping governor meridian combined with rehabilitation training cloud improve the neurological function of patients and alleviate the impairment of vascular function, thus better improve the muscle tone of affected limbs with spasticity after stroke, relieve spasticity of affected limbs, and improve motor function and daily living ability.
