通窍化栓汤联合银杏内酯注射液对风痰瘀阻型急性脑梗死患者脑血管血流动力学及凝血功能的影响

摘　　要：目的 探讨通窍化栓汤联合银杏内酯注射液治疗风痰瘀阻型急性脑梗死患者的临床效果。方法 选取2018年12月至2020年12月收治的70例风痰瘀阻型急性脑梗死患者作为研究对象,以随机数字表法将其分为对照组和观察组,各35例。对照组采用常规治疗+银杏内酯注射液,观察组在对照组基础上联合通窍化栓汤。比较两组的脑血管血流动力学指标、凝血功能指标、ET、NO及血清相关因子水平。结果 治疗后,观察组的Vm、Vp高于对照组,DVm、DVp低于对照组(P<0.05)。治疗后,观察组的R、K长于对照组,MA、α角小于对照组(P<0.05)。治疗后,观察组的ET水平低于对照组,NO水平高于对照组(P<0.05)。治疗后,观察组的GMP-140、PAF、HMGB1、MMP-9水平低于对照组(P<0.05)。结论 通窍化栓汤联合银杏内酯注射液可改善风痰瘀阻型急性脑梗死患者的脑血管血流动力学、凝血功能及ET、NO水平,降低GMP-140、PAF、HMGB1、MMP-9水平。Objective To explore the clinical effect of Tongqiao Huashuan decoction combined with ginkgolide injection in the treatment of patients with acute cerebral infarction of wind phlegm and blood stasis type.Methods A total of 70patients with acute cerebral infarction of wind phlegm and blood stasis type treated from December 2018 to December 2020were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 35 cases in each group.The control group used routine treatment + ginkgolide injection,and the observation group combined with Tongqiao Huashuan decoction on the basis of the control group.The cerebrovascular hemodynamics indexes,coagulation function indexes,ET,NO and serum related factors levels were compared between the two groups.Results After treatment,Vm and Vp in the observation group were higher than those in the control group,DVm and DVp were lower than those in the control group(P <0.05).After treatment,R and K in the observation group were longer than those in the control group,MA and α-angle were smaller than those in the control group(P <0.05).After treatment,the level of ET in the observation group was lower than that in the control group,the level of NO was higher than that in the control group(P <0.05).After treatment,the levels of GMP-140,PAF,HMGB1 and MMP-9 in the observation group were lower than those in the control group(P<0.05).Conclusion Tongqiao Huashuan decoction combined with ginkgolide injection can improve the cerebrovascular hemodynamics,coagulation function and the levels of ET and NO,and reduce the levels of GMP-140,PAF,HMGB1 and MMP-9 in patients with acute cerebral infarction of wind phlegm and blood stasis type.
