摘  要:目的:探究分析头高位预吸氧在肥胖患者无痛人工流产术中的应用效果。方法:选取2019年5月-2021年5月北京市西城区展览路医院进行无痛人工流产手术的肥胖患者100例,随机分为对照组与试验组,各50例。对照组在手术前面罩吸氧5 L/min,共3 min,手术中持续面罩吸氧2 L/min。试验组术前不给予预吸氧,但持续术中面罩吸氧2 L/min。观察两组患者术中血压、血氧饱和度、心率、不良反应情况。结果:两组患者麻醉后血压、血氧饱和度、心率均低于麻醉前,但差异无统计学意义(P>0.05)。麻醉后,试验组血压、心率均显著高于对照组,血氧饱和度显著低于对照组,差异有统计学意义(P<0.05)。对照组不良反应发生率为4.0%,显著低于试验组的14.0%,两组比较差异有统计学意义(P<0.05)。结论:肥胖者在进行无痛人工流产术时术前给予预吸氧,能够有效提高其术中的氧气储备含量,有效保证手术安全性,最大程度减少不良反应发生。Objective: To explore and analyze the application effect of pre-inhalation oxygen in reverse trendelenburg position in painless induced abortion of obese patients. Methods: A total of 100 obese patients who underwent painless induced abortion in Exhibition Road Hospital in Xicheng District of Beijing from May 2019 to May 2021 were selected and randomly divided into control group and experimental group, with 50 patients in each group. The control group was given oxygen inhalation at 5 L/min for3 minutes using facial mask before surgery, the facial mask inhalation was continued during surgery at 2 L/min. In the experimental group, no pre-inhalation oxygen was given, but the facial mask inhalation was continued during surgery at 2 L/min.The blood pressure, blood oxygen saturation, heart rate and adverse reactions of the two groups were observed. Results: The blood pressure, blood oxygen saturation and heart rate of patients in the two groups after anesthesia were lower than those before anesthesia, but the difference was not statistically significant(P>0.05). After anesthesia, the blood pressure and heart rate of the experimental group were significantly higher than those of the control group, and the blood oxygen saturation was significantly lower than that of the control group, and the differences were statistically significant(P<0.05). The incidence of adverse reactions in the control group was 4.0%, which was significantly lower than 14.0% in the experimental group, and the difference between the two groups was statistically significant(P<0.05). Conclusion: Pre-inhalation oxygen in painless induced abortion of obese patients can effectively increase the oxygen reserve during the operation, effective ensure the surgery safety, and maximize the reduction of adverse reactions.
