通腑益肺汤治疗痰热壅肺型重症肺炎合并Ⅱ型呼吸衰竭的疗效观察

摘　　要：目的:探讨应用自拟通腑益肺汤治疗痰热壅肺型重症肺炎合并Ⅱ型呼吸衰竭的临床疗效。方法:将2020年1月-2021年1月北京市昌平区中医医院收治的80例痰热壅肺型重症肺炎合并Ⅱ型呼吸衰竭患者作为研究对象,随机分为试验组与对照组,各40例。对照组患者应用喜炎平注射液治疗,试验组在对照组基础上应用自拟通腑益肺汤。观察两组患者临床疗效。结果:两组患者治疗后试验组总有效率明显高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后胸闷胸痛、气促、发热、咳黄脓痰、神昏烦躁中医证候积分均较治疗前降低(P<0.05),且试验组各项中医证候积分显著低于对照组(P<0.05)。试验组治疗后各项肺功能指标水平改善情况均优于对照组(P<0.05)。结论:应用自拟通腑益肺汤治疗痰热壅肺型重症肺炎合并Ⅱ型呼吸衰竭患者效果满意,可显著缓解临床症状,改善肺功能。Objective: To explore the clinical efficacy of self-made Tongfu Yifei Decoction in the treatment of severe pneumonia with phlegm-heat obstructing lung type and type II respiratory failure. Methods: Eighty patients of severe pneumonia with phlegmheat obstructing lung type and type II respiratory failure treated in the Hospital of Traditional Chinese Medicine in Changping District from January 2020 to January 2021 were selected as the subjects, and they were randomly divided into experimental group and control group with 40 cases in each group. The control group was treated with Xiyanping Injection and the experimental group was treated with self-made Tongfu Yifei Decoction on the basis of control group. The clinical efficacy of the two groups was observed. Results: After treatment, the total effective rate of the experimental group was significantly higher than that of the control group, and the difference was statistically significant(P<0.05). After treatment, the scores of TCM syndromes in the two groups of patients for chest tightness, chest pain, shortness of breath, fever, coughing yellow pus and sputum, unconsciousness and dysphoria were all lower than those before treatment(P<0.05), and the scores of TCM syndromes in experimental group were significantly lower than those in control group(P<0.05). After treatment, the improvement levels of each pulmonary function index in the experimental group were better than those in the control group(P<0.05). Conclusion: The application effect of self-made Tongfu Yifei Decoction is satisfactory in treating severe pneumonia with phlegm-heat obstructing lung type and type II respiratory failure, which can significantly relieve the symptoms of patients and improve lung function.
