摘  要:目的观察急性心肌梗死(AMI)病人入院血清微小RNA-126(miR-126)、去整合素-金属蛋白酶9(ADAM9)的表达水平,并探讨急性心肌梗死病人入院血清miR-126、ADAM9表达水平与急性心肌梗死短期预后的相关性。方法选取2017年6月—2019年6月在安徽医科大学第二附属医院急诊内科确诊并收治的急性心肌梗死病人142例作为观察组,选取同期体检健康者140名作为对照组。1个月后观察组复查心脏彩超,根据其是否发生不良心血管事件(MACEs)分为MACEs组(36例),未发生MACEs组(106例)。实时荧光定量聚合酶链式反应(qRT-PCR)测定急性心肌梗死病人血清中miR-126表达水平;酶联免疫吸附实验(ELISA)法检测急性心肌梗死病人ADAM9水平;Pearson分析急性心肌梗死病人入院血清miR-126、ADAM9表达的相关性;受试者工作特征曲线(ROC)分析血清miR-126、ADAM9表达水平对急性心肌梗死病人发生MACEs的预测价值。结果观察组高血压比例、体质指数(BMI)均高于对照组(P<0.05);观察组血清中miR-126表达水平低于对照组,ADAM9表达水平高于对照组(P<0.05);观察组病人血清miR-126与ADAM9表达水平呈负相关(r=-0.580,P<0.001);MACEs组血清miR-126表达水平低于未发生MACEs组,ADAM9表达水平高于未发生MACEs组(P<0.05);ROC曲线显示miR-126、ADAM9联合预测急性心肌梗死病人发生MACEs的ROC曲线下面积为0.786[95%CI(0.700,0.873)],敏感度为86.1%,特异性为77.9%。结论急性心肌梗死病人血清中miR-126呈低表达、ADAM9呈高表达,两者呈负相关,两者联合检测对预测急性心肌梗死病人短期内发生MACEs有一定价值。Objective To observe the expression levels of microRNA-126(miR-126)and a disintegrin and metalloprotease 9(ADAM9)in patients with acute myocardial infarction(AMI),and to explore the correlation between the expression levels of miR-126 and ADAM9 in patients with AMI and the short-term prognosis.Methods A total of 142 patients with AMI were selected as the observation group,and 140 healthy people during the same period were selected as the control group.One month later,the observation group was re-examined by cardiac color Doppler echocardiography.According to the occurrence of adverse cardiovascular events(MACEs),the patients were divided into MACEs group(36 cases)and non-MACES group(106 cases).The expression level of miR-126 in serum of patients with AMI was determined by real-time fluorescence quantitative PCR(qRT-PCR).ADAM9 level was detected by enzyme-linked immunosorbent assay(ELISA).The correlation of miR-126 and ADAM9 expression in serum were analysed.The prediction value of serum miR-126 and ADAM9 expression levels was analyzed by receiver operating characteristic curve(ROC).Results The proportion of hypertension and BMI in observation group were higher than those in the control group(P<0.05).The expression level of miR-126 in serum of the observation group was lower than that of the control group,and the expression level of ADAM9 was higher than that of the control group(P<0.05).The expression level of miR-126 and ADAM9 in serum of the observation group was negatively correlated(r=-0.580,P<0.001).The expression level of miR-126 in serum in MACEs group was lower than that in the non-MACES group,and the expression level of ADAM9 was higher than that in the non-MACES group(P<0.05).ROC curve showed that the area under ROC curve of miR-126 and ADAM9 combined to predict MACEs in patients with AMI was 0.786[95%CI(0.700,0.873)],with sensitivity of 86.1%and specificity of 77.9%.Conclusion The expression of miR-126 was low and ADAM9 was high in serum of AMI patients with a negative relationship.The combined detecti
