摘 要:目的:调查陕西地区孤独症谱系障碍(ASD)患儿的25-羟维生素D[25(OH)D]水平分布特点,以期为ASD患儿临床诊治提供依据。方法:选取初诊来自陕西地区的2~8岁ASD患儿380例为ASD组,同期体检的同龄健康儿童385例作为对照组,采用电化学发光法对两组儿童进行血清25(OH)D水平检测。同时对不同年龄、不同性别及全年各季节的ASD患儿25(OH)D不足及缺乏率进行分析。结果:ASD组患儿25(OH)D水平(36.39±12.9)ng/ml显著低于健康对照组(39.98±12.4)ng/ml(t=3.921,P<0.01),ASD组患儿25(OH)D不足及缺乏率(34.5%)显著高于对照组(21.8%)(χ^(2)=15.159,P<0.01)。ASD组患儿3~6岁及6~8岁年龄组25(OH)D不足及缺乏率(52.3%、58.8%)明显高于2~3岁年龄组(19.8%)(P<0.01);ASD组患儿春季、冬季25(OH)D不足及缺乏率(38.2%、47%)显著高于夏季(18.5%)(P<0.01);ASD患儿不同性别25(OH)D不足及缺乏率比较差异无统计学意义(P>0.05)。结论:陕西地区ASD患儿25(OH)D水平明显低于健康儿童,其中学龄前期、学龄期以及冬季、春季25(OH)D不足及缺乏更显著。因此对ASD患儿应加强25(OH)D的营养管理。Objective:To understand the distribution characteristic of 25-hydroxyvitamin D[25(OH)D]level in children with autism spectrum disorder(ASD)in Shaanxi,so as to provide basis for clinical diagnosis and treatment of ASD.Methods:A total of 380 ASD children aged 2-8 years in Shaanxi were selected as ASD group,and a total of 385 healthy children of the same age were selected as control group.Serum 25(OH)D level was detected by electrochemiluminescence.The rate of 25(OH)D insufficiency and deficiency in ASD children of different ages,genders and seasons was analyzed.Results:The level of 25(OH)D in ASD group was significantly lower than that in healthy control group[(36.39±12.9)ng/ml vs.(39.98±12.4)ng/ml,t=3.921,P<0.01].The insufficiency and deficiency rate of 25(OH)D in ASD group was significantly higher than that in control group(34.5%vs.21.8%,χ^(2)=15.159,P<0.01).The insufficiency and deficiency rate in the age groups with 3-6 and 6-8 years(52.3%,58.8%)in ASD group were significantly higher than that(19.8%)in the age groups with 2-3 years(all P<0.01).The insufficiency and deficiency rate in spring and winter(38.2%,47.0%)in ASD group were significantly higher than that(18.5%)in summer(all P<0.01).There was no significant difference in 25(OH)D insufficiency and deficiency rate between different genders in ASD children(all P>0.05).Conclusion:In Shaanxi,the level of 25(OH)D in ASD children is significantly lower than that in healthy children,and the insufficiency and deficiency in pre-school,school-age,winter and spring are more significant.Therefore,the nutritional management of 25(OH)D should be strengthened in ASD children.
