日常胎动记录图和评估脐带绕颈对胎儿宫内安危的价值研究

摘　　要：目的分析日常胎动记录图和评估脐带绕颈对胎儿宫内安危的应用价值。方法选取我院2019年12月-2020年12月共128例足月分娩孕妇作为研究对象,根据分娩结局分为宫内安全组(n=55)和宫内不安全组(n=73),分析日常胎动记录图和评估脐带绕颈对胎儿宫内安危的价值。结果128例足月分娩孕妇入院后均接受自然分娩建议,分娩中转剖宫产46例,临产后选择剖宫产6例。两组S/D、脐带绕颈周数、FM、Fischer评分、分娩方式比较,差异有统计学意义(P<0.05);两组年龄、AFI比较,差异无统计学意义(P>0.05);多因素Logistic回归分析显示,FM异常、FM减少或消失以及剖宫产,均是影响脐带绕颈胎儿宫内安危的高危因素(P<0.05);日常胎动记录图预测灵敏度为87.27%,特异度为90.00%。结论日常胎动记录图在评估脐带绕颈胎儿宫内安危中具有较高的应用价值,能够有效预测胎儿的状态,有利于指导临床采取针对性的分娩方式。Objective To analyze the daily fetal movement records and evaluate the application value of the umbilical cord around neck on fetal safety in the uterus.Methods A total of 128 full-term pregnant women in our hospital from December 2019 to December 2020 were selected as the research objects.According to the delivery outcome,they were divided into an intrauterine safety group(n=55)and an intrauterine unsafe group(n=73).The daily fetal movement records and evaluate the value of the umbilical cord around neck on the fetal safety in the uterus were evaluated.Results A total of 128 cases of full-term delivery of pregnant women after admission were accepted natural childbirth Suggestions,46 cases of delivery to cesarean section,6 cases of cesarean section after labor.There were statistically significant differences in S/D,umbilical cord circumference,FM,Fischer score and delivery mode between the two groups(P<0.05).There was no significant difference in age and AFI between the two groups(P>0.05).Multivariate logistic regression analysis showed that abnormality of FM,reduction or disappearance of FM,and cesarean section were all high-risk factors affecting the intrauterine safety of fetus with umbilical cord around neck(P<0.05).The prediction sensitivity and specificity of daily fetal movement record were 87.27%and 90.00%,respectively.Conclusion The daily fetal movement record has high application value in evaluating the intrauterine safety of the fetus with the umbilical cord around neck.,which can effectively predict the state of the fetus and is helpful to guide the clinical adoption of targeted delivery methods.
