八段锦防治绝经后女性骨质疏松的Meta分析

摘　　要：目的:评价八段锦防治绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)的疗效。方法:检索CNKI、SinoMed、VIP、WanFang Data、PubMed、Embase、Cochrane Library、EBSCO、Web of Science、Ovid数据库中关于八段锦防治PMOP疗效的随机对照试验(randomized controlled trials,RCT),使用Revman5.4.1进行Meta分析。结果:纳入RCT文献11篇,共759例样本量。Meta分析显示:与对照组相比,八段锦在改善腰椎L_(2)~L_(4)骨密度(BMD)、血钙(Ca^(2+))水平、血清骨钙素(BGP)水平、尿脱氧吡啶啉排泄率(DPD/Cr)、腰背疼痛及提高防治PMOP的有效率上具有积极作用,且差异有统计学意义(P<0.05);亚组分析显示,经八段锦干预≤6个月的L_(2)~L_(4)BMD差异有统计学意义(P<0.05),干预12个月后差异无统计学意义(P>0.05),敏感性分析剔除2项研究后,差异有统计学意义(P<0.05);描述性分析显示,八段锦能提高绝经后女性的平衡能力;在左股骨颈BMD、血磷(P)及血清碱性磷酸酶(ALP)上,两组差异无统计学意义(P>0.05)。结论:八段锦对防治PMOP能起到积极作用,应推广应用。Objective:To evaluate the effect of Baduanjin on postmenopausal osteoporosis(PMOP).Methods:Randomized controlled trials(RCT)on the effect of Baduanjin in the prevention and treatment of PMOP were searched from CNKI,SinoMed,VIP,Wanfang Data,PubMed,Embase,Cochrane Library,EBSCO,Web of Science and OVID databases.Meta analysis were performed using RevMan5.4.1.Results:Eleven RCTSs were included,with a total sample size of 759 cases.Meta-analysis showed that compared with the control group,Baduanjin had a positive effect on improving lumbar L_(2)~L_(4) bone mineral density(BMD),serum calcium(Ca^(2+))levels,serum osteocalcin(BGP)levels,urinary deoxypyridine excretion rate(DPD/CR),back pain and improving the effective rate on prevention and treatment of PMOP.The difference was statistically significant(P<0.05).Subgroup analysis showed that the difference of L_(2)~L_(4) BMD≤6months after Baduanjin intervention was statistically significant(P<0.05),but there was no statistically significant difference after 12 months'intervention(P>0.05).Sensitivity analysis showed that the difference was statistically significant after excluding 2 studies(P<0.05).Descriptive analysis showed that Baduanjin could improve the balance ability of postmenopausal women.There was no significant difference in left femoral neck BMD,serum phosphorus(P)and serum alkaline phosphatase(ALP)between the two groups(P>0.05).Conclusion:Baduanjin plays an active role in the prevention and treatment of PMOP and should be promoted.
