摘  要:目的 观察特发性肺纤维化(idiopathic pulmonary fibrosis,IPF)患者血清微RNA-29b(mirco RNA-29b,miR-29b)表达变化,并分析其与肺动脉高压(pulmonary hypertension,PH)的关系。方法 选取河北省胸科医院2018年3月至2021年5月收治的116例初诊IPF患者作为IPF组,同期招募112例健康志愿者作为健康对照组。两组均取外周血并采用实时荧光定量PCR检测血清miR-29b表达水平并进行比较。IPF组患者根据是否并发PH分为PH组(并发PH)和对照组(未并发PH),比较两组血清miR-29b表达水平,并采用logistic多元回归分析IPF患者发生PH的危险因素。结果 IPF组血清miR-29b表达水平低于健康对照组,差异有显著性(P<0.05);IPF组PH发生率为32.76%,且PH组血清miR-29b表达水平低于对照组(P<0.05)。Logistic多元回归分析显示,一氧化碳弥散量、血清miR-29b表达水平是IPF患者发生PH的危险因素(OR=5.248、5.551,P<0.05)。结论 IPF患者血清miR-29b表达水平下降,且发生PH的患者表达水平更低,一氧化碳弥散量、血清miR-29b表达水平均是IPF发生PH的危险因素。Objective To observe the expression change of serum microRNA-29b(miR-29b)in patients with idiopathic pulmonary fibrosis(IPF)and analyze its relationship with pulmonary hypertension(PH).Method 116 newly diagnosed IPF patients in Hebei Chest Hospital from March 2018 to May 2021 were selected,and 112 healthy volunteers were recruited in the same period,which were recorded as IPF group and healthy group respectively.Peripheral blood was taken from both groups,then the expression of serum miR-29b was detected by real-time fluorescence quantitative reverse transcription polymerase chain reaction(RT-qPCR)and was compared.According to the presence or absence of PH,the IPF group was divided into PH occurrence group and PH non occurrence group,and the expression levels of miR-29b in two groups were compared.Logistic multiple regression analysis method was used to clarify the risk factors for the occurrence of PH in IPF group.Result The expression level of serum miR-29b in the IPF group was lower than that in the healthy group(P<0.05).The incidence of PH in the IPF group was 32.76%,and the expression level of serum miR-29b in the PH group was lower than that in the non-occurring group(P<0.05),carbon monoxide diffusion and serum miR-29b expression levels were both risk factors in the IPF group with PH(OR=5.248,5.551,P<0.05).Conclusion The expression level of serum miR-29b in patients with IPF decreases,and the expression level in patients with PH is lower.The dispersion of carbon monoxide and serum miR-29b expression are both risk factors for the occurrence of PH in IPF.
