芪骨胶囊联合PKP治疗肝肾不足型绝经后骨质疏松性椎体压缩性骨折的效果

摘　　要：目的:观察芪骨胶囊联合经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗肝肾不足型绝经后骨质疏松性椎体压缩性骨折(osteoporosis vertebral compression fractures,OVCF)患者的效果。方法:选取厦门市中医院2019年1月-2020年12月96例肝肾不足型绝经后OVCF患者作为研究对象。根据抽签法将其分为对照组和观察组,各48例。两组均给予PKP手术治疗,观察组在术后3 d口服芪骨胶囊,两组术后每天常规补充钙剂。比较两组术前,术后1周、3个月疼痛、功能障碍程度,术前及术后3个月骨代谢指标。结果:术后1周,两组视觉模拟量表(VAS)评分比较差异无统计学意义(P>0.05),观察组术后3个月VAS、Oswestry功能障碍指数(ODI)评分均低于对照组,术后1周ODI评分低于对照组(P<0.05)。术前两组β-胶原特殊系列(β-crosslaps)、总Ⅰ型胶原氨基端延长肽(total-PⅠNP)、血清骨钙素(OC)水平比较差异均无统计学意义(P>0.05),术后3个月,观察组β-crosslaps水平较对照组更低,total-PⅠNP、OC水平均较对照组更高(P<0.05)。结论:芪骨胶囊联合PKP手术治疗肝肾不足型绝经后OVCF能减轻患者疼痛程度,并促进骨折愈合和肢体功能恢复。Objective:To observe the efficacy of Qigu Capsule combined with percutaneous kyphoplasty(PKP)in the treatment of postmenopausal osteoporotic vertebral compression fractures(OVCF)with liver and kidney insufficiency.Method:A total of 96 cases of postmenopausal OVCF patients with liver and kidney insufficiency admitted to Xiamen Hospital of Traditional Chinese Medicine from January 2019 to December 2020 were selected as the research objects,and they were divided into control group and observation group by lottery method,48 cases in each group.Both groups were given the PKP surgery,the observation group was orally treated with Qigu Capsule 3 days after surgery,and the two groups were routinely supplemented with calcium every day after surgery.The degree of pain and dysfunction before surgery,1 week and 3 months after surgery of the two groups were compared,and bone metabolism indexes before surgery and 3 months after surgery of the two groups were compared.Result:There was no significant difference in visual analog scale(VAS)scores between the two groups 1 week after surgery(P>0.05).VAS and Oswestry dysfunction index(ODI)scores of the observation group 3 months after surgery were lower than those of the control group,and ODI score of the observation group 1 week after surgery was lower than that of the control group(P<0.05).There were no significant differences in the levels ofβ-crosslaps,total procollagen type I N-terminal propeptide(total-PⅠNP)and serum osteocalcin(OC)between the two groups before surgery(P>0.05).Three months after surgery,the level ofβ-crosslaps in observation group was lower than that in control group,the levels of total-PⅠNP and OC were higher than those of the control group(P<0.05).Conclusion:Qigu Capsule combined with PKP surgery for the treatment of postmenopausal OVCF with liver and kidney insufficiency can reduce the pain of patients,and promote fracture healing and limb function recovery.
