不同病情程度ICP孕妇血清GCA、ALT、AST、TBIL水平及胎盘HIF-1α蛋白表达情况观察

摘　　要：目的:探讨不同病情程度妊娠期肝内胆汁淤积症(ICP)孕妇血清甘氨胆酸(GCA)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)水平及胎盘缺氧诱导因子-1α(HIF-1α)蛋白表达情况。方法:回顾性选择东莞市第八人民医院2020年3-12月收治的62例ICP孕妇作为观察组,其中轻度40例,重度22例,并纳入同期62例健康孕妇作为对照组。比较两组的肝胆血清指标水平与胎盘HIF-1α蛋白表达情况,并对比不同病情程度ICP孕妇肝胆血清指标水平及胎盘HIF-1α蛋白表达情况。结果:观察组的ICP、GCA、ALT、AST及TBIL水平均高于对照组(P<0.05)。观察组的HIF-1α蛋白表达总阳性率为72.58%,高于对照组的53.23%(P<0.05)。重度ICP孕妇的GCA、ALT、AST及TBIL水平均高于轻度ICP孕妇(P<0.05)。重度ICP孕妇的HIF-1α蛋白表达阳性率为94.45%,高于轻度ICP孕妇的60.00%(P<0.05)。结论:ICP孕妇的胆汁酸、肝酶水平及胎盘HIF-1α蛋白表达较正常孕妇高,重度ICP孕妇的胎盘HIF-1α蛋白表达高于轻度孕妇。Objective:To investigate the serum glycocholic acid(GCA),alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL)and placental hypoxia-inducible factor-1α(HIF-1α)protein expression in pregnant women with intrahepatic cholestasis of pregnancy(ICP)of different severity levels.Method:Retrospectively,62 pregnant women with ICP admitted to Dongguan Eighth People’s Hospital from March to December 2020 were selected as the observation group,including 40 cases of mild ICP and 22 cases of severe ICP,and 62 cases of healthy pregnant women in the same period were included as the control group.The hepatobiliary serum index levels and placental HIF-1αprotein expression were compared between the two groups,and the hepatobiliary serum index levels and placental HIF-1αprotein expression in pregnant women with different degrees of ICP were compared.Result:The levels of ICP,GCA,ALT,AST and TBIL in the observation group were higher than those in the control group(P<0.05).The total positive rate placental of HIF-1αprotein expression in the observation group was 72.58%,which was higher than 53.23%in the control group(P<0.05).The levels of GCA,ALT,AST and TBIL in pregnant women with severe ICP were higher than those in pregnant women with mild ICP(P<0.05).The positive rate of placental HIF-1αprotein expression in pregnant women with severe ICP was 94.45%,which was higher than 60.00%in pregnant women with mild ICP(P<0.05).Conclusion:The levels of bile acids,liver enzymes and placental HIF-1αprotein expression in pregnant women with ICP are higher than those in normal pregnant women,and the expression of placental HIF-1αprotein in pregnant women with severe ICP is higher than that in pregnant women with mild ICP.
