逆行输尿管软镜下钬激光碎石术治疗单发直径1.5~2.0 cm肾盂结石的效果

摘　　要：目的:分析逆行输尿管软镜下钬激光碎石术治疗单发直径1.5~2.0 cm肾盂结石患者的效果。方法:选取厦门市中医院2018年4月-2021年3月肾盂结石患者95例,根据手术方案不同分为观察组(n=48)、对照组(n=47),对照组行经皮肾镜取石术,观察组行逆行输尿管软镜下钬激光碎石术,比较两组手术相关指标(术中出血量、手术时间、住院时间)、一次性结石清除率、术后并发症发生情况(发热、感染、血尿)。结果:观察组术中出血量较对照组少,手术时间较对照组长,住院时间较对照组短(P<0.05);观察组一次性结石清除率的93.75%(45/48)与对照组的89.36%(42/47)比较,差异无统计学意义(P>0.05);观察组术后并发症发生率为4.17%(2/48),较对照组的19.15%(9/47)低(P<0.05)。结论:逆行输尿管软镜下钬激光碎石术治疗单发直径1.5~2.0 cm肾盂结石患者具有较好效果,虽在手术时间方面不具有优势,但可减少手术创伤,缩短术后康复进程,降低术后并发症发生率,且一次性结石清除率高,安全可靠。Objective:To analyze the effect of retrograde soft ureteroscopic holmium laser lithotripsy in the treatment of single pyelolithiasis of diameter 1.5-2.0 cm.Method:A total of 95 patients with pyelolithiasis in Xiamen Hospital of Traditional Chinese Medicine from April 2018 to March 2021 were selected and divided into observation group(n=48)and control group(n=47)according to different surgical plans.Percutaneous nephrolithotomy was performed in the control group,and retrograde soft ureteroscopic holmium laser lithotripsy was performed in the observation group.Operation related indicators(intraoperative blood loss,operation time,length of hospital stay),rate of one-time stone removal,postoperative complications(fever,infection,hematuria)were compared between the two groups.Result:The observation group had less intraoperative blood loss,longer operation time and shorter hospital stay than those of the control group(P<0.05).There was no significant difference in the rate of one-time stone removal between the observation group(93.75%,45/48)and the control group(89.36%,42/47)(P>0.05).The incidence of postoperative complications in the observation group was 4.17%(2/48),which was lower than 19.15%(9/47)in the control group(P<0.05).Conclusion:Retrograde soft ureteroscopic holmium laser lithotripsy has a good effect in the treatment of single pyelolithiasis of diameter 1.5-2.0 cm.Although it has no advantage on operation time,it can reduce surgical trauma,shorten the postoperative rehabilitation process,reduce the incidence of postoperative complications,and has a high rate of one-time stone removal rate,which is safe and reliable.
