半椎板切除自体髂骨取骨植骨术中应用S-100可吸收止血绫化骨蜡封堵骨缝止血的效果

摘　　要：目的:评价在半椎板切除自体髂骨取骨植骨术中应用S-100可吸收止血绫化骨蜡封堵骨缝进行止血的效果。方法:收集2018年1月-2020年6月在贵州医科大学附属肿瘤医院行半椎板切除自体髂骨取骨植骨手术的62例患者临床资料进行回顾性分析。观察组37例在半椎板切除自体髂骨取骨手术中使用S-100可吸收止血绫将其水化成骨蜡封堵骨缝进行止血,对照组25例采用常规的术中止血措施。比较两组术中出血量、下床活动时间、住院时间、术后并发症(血肿、感染、出血)。结果:观察组术中出血量、下床活动时间及住院时间均少于对照组(P<0.05)。两组均无因术中出血多而需输血的病例。两组均无术后感染、术后血肿的发生,观察组术后出血发生率低于对照组,两组并发症发生率比较差异无统计学意义(P>0.05)。结论:在行半椎板切除自体髂骨取骨植骨手术中应用S-100可吸收止血绫化骨蜡封堵骨缝止血的效果显著。Objective:To evaluate the hemostatic effect of S-100 absorbable hemostatic silk bone wax blocking bone suture in hemilaminectomy and autologous iliac bone grafting.Method:The clinical data of 62 patients who underwent hemilaminectomy and autologous iliac bone grafting in Affiliated Tumor Hospital of Guizhou Medical University from January 2018 to June 2020 were collected and analyzed retrospectively.Among them,37 cases(named as the observation group)were treated by S-100 absorbable hemostatic silk,which was hydrated it into bone wax to blocked the bone suture for hemostasis during hemilaminectomy and autologous iliac bone osteotomy,and 25 cases(named as the control group)were treated by routine intraoperative hemostasis measures.The amount of intraoperative bleeding,out of bed activity time,hospital stay,postoperative complications(hematoma,infection,bleeding)were compared between the two groups.Result:The amount of intraoperative bleeding,out of bed activity time and hospital stay in the observation group were less than those in the control group(P<0.05).There were no cases requiring blood transfusion due to excessive intraoperative bleeding in both groups.There was no postoperative infection or postoperative hematoma in both groups.There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:The application of S-100 absorbable hemostatic silk bone wax to block bone suture in hemilaminectomy and autologous iliac bone grafting has a significant hemostatic effect.
