小剂量地高辛联合芪参益气滴丸治疗低体重慢性心力衰竭患者的效果观察

摘　　要：目的:观察小剂量地高辛联合芪参益气滴丸治疗低体重慢性心力衰竭患者的效果。方法:选取2019年3月-2020年7月就诊于武威市凉州医院的经标准抗心衰治疗(应用了利尿剂、血管紧张素转化酶抑制剂/血管紧张素Ⅱ受体阻滞剂/血管紧张素受体脑啡肽酶抑制、β受体阻滞剂和醛固酮受体拮抗剂)后心功能仍为Ⅱ、Ⅲ级的低体重慢性心力衰竭患者共60例。采用随机法将患者分为对照组(30例)和研究组(30例)。对照组给予常规剂量地高辛(0.25 mg/次,1次/d)口服治疗;研究组给予小剂量地高辛口服(0.125 mg/次,1次/d)联合芪参益气滴丸(0.5 g/次,3次/d)口服治疗,两组均连续治疗4周。在研究期间记录两组治疗前和治疗后3、6、12个月的脑利尿钠肽(BNP)、左心室射血分数(LVEF)水平,治疗后1年内再住院率及地高辛中毒事件发生率。结果:对照组发生1例地高辛中毒事件,终止研究;研究组未发生地高辛中毒,两组地高辛中毒事件发生率比较,差异无统计学意义(P>0.05)。治疗后3、6、12个月,研究组LVEF均高于对照组,BNP水平均低于对照组,差异有统计学意义(P<0.05)。研究组治疗后1年内再住院率为20.0%,明显低于对照组的44.8%,差异有统计学意义(P<0.05)。结论:小剂量地高辛联合芪参益气滴丸治疗低体重慢性心力衰竭患者临床效果良好,安全风险低,在临床上值得推广应用。Objective:To observe the efficacy of low dose Digoxin combined with Qishen Yiqi Dropping Pills in the treatment of low weight patients with chronic heart failure.Method:From March 2019 to July 2020,a total of 60 patients with low weight chronic heart failure whose heart function was still GradeⅡorⅢafter standard anti-heart failure treatment(diuretic,angiotensin converting enzyme inhibitor/angiotensinⅡreceptor blocker/angiotensinⅡreceptor enkephalinase inhibitor,βreceptor blocker and aldosterone receptor antagonist applied)in Wuwei Liangzhou Hospital were selected.Patients were randomly divided into the control group(30 cases)and the study group(30 cases).The control group was given a conventional dose of Digoxin(0.25 mg/time,once a day)oral treatment,the study group was given low dose Digoxin oral(0.125 mg/time,once a day)combined with Qishen Yiqi Dropping Pills(0.5 g/time,3 times a day),both were treated for 4 consecutive weeks.During the study period,the level of brain natriuretic peptide(BNP),left ventricular ejection fraction(LVEF)before treatment and 3,6 and 12 months after treatment,the rate of rehospitalization within one year after treatment and the incidence of Digoxin poisoning were recorded in the two groups.Result:One case of Digoxin poisoning occurred in the control group,and the study was terminated.There was no Digoxin poisoning in the study group,there was no significant difference(P>0.05).At 3,6 and 12 months after treatment,the LVEF of the study group was higher than that of the control group,and BNP level was lower than that of the control group,all with statistical significance(P<0.05).The rehospitalization rate of the study group within one year after treatment was 20.0%,which was significantly lower than 44.8%of the control group,and the difference was statistically significant(P<0.05).Conclusion:Low dose Digoxin combined with Qishen Yiqi Dropping Pills is well used in treating patients with low weight chronic heart failure with good clinical effect and low safety risk,which is wort
