快速康复外科模式在关节镜下治疗肩袖损伤患者围手术期中的应用

摘　　要：目的:探讨快速康复外科模式在关节镜下治疗肩袖损伤患者围手术期中的应用价值。方法:纳入2019年4月-2021年3月在南京明基医院接受治疗的86例肩袖损伤患者进行研究,所有患者均接受关节镜治疗。按随机数字表法将患者分为两组,各43例。其中对照组采用传统康复模式干预;观察组采用快速康复外科模式干预。观察两组住院情况、疼痛情况、肩关节功能、生活质量及并发症发生情况。结果:观察组住院时间短于对照组,住院费用少于对照组(P<0.05)。术后1周,观察组视觉模拟评分法(VAS)评分明显低于对照组(P<0.05)。术后1个月,观察组肩关节疼痛、功能活动、活动度及肌力情况评分均高于对照组(P<0.05)。术后1个月,观察组生理职能、生理功能、总体健康、躯体疼痛、情感职能、社会功能、精神健康及活力评分均高于对照组(P<0.05)。观察组并发症发生率(2.33%)明显低于对照组(18.60%),差异有统计学意义(P<0.05)。结论:快速康复外科模式可有效缩短肩袖损伤关节镜治疗患者的住院时间,减少住院费用,促进患者预后,缓解疼痛,改善关节功能,提高生活质量,减少并发症发生,值得临床应用及推广。Objective:To explore the value of rapid rehabilitation surgery model in arthroscopic treatment of patients with rotator cuff injury during perioperative period.Method:A total of 86 rotator cuff injury patients of undergoing the arthroscopic treatment from April 2019 to March 2021 in Nanjing BenQ Hospital were chosen as research subjects.According to the random number table,the patients were divided into two groups,with 43 cases in each group.The control group was intervened with the traditional rehabilitation model.The intervention of rapid rehabilitation surgery mode was adopted in the observation group.The hospitalization conditions,pain conditions,shoulder joint function,quality of life and complications of the two groups were observed.Result:The hospitalization time in the observation group was shorter than that of the control group,the hospitalization costs was lower than that of the control group,the differences were statistically significant(P<0.05).Two weeks postoperatively,the visual analogue scale(VAS)scores of the observation group was significantly lower than that of the control group(P<0.05).One month after surgery,the scores of shoulder pain,functional activity,range of motion and muscle strength in the observation group were higher than those in the control group(P<0.05).One month after surgery,the scores of physiological function,physiological function,overall health,physical pain,emotional function,social function,mental health and vitality in the observation group were higher than those in the control group(P<0.05).The incidence of complications in the observation group(2.33%)was significantly lower than 18.60%in the control group(P<0.05).Conclusion:The rapid rehabilitation surgery model can effectively shorten the hospitalization time of patients with rotator cuff injury treated by arthroscopy,reduce the hospitalization costs,promote the prognosis of patients,relieve pain,improve joint function,improve the quality of life,and reduce the complications,which is worthy of clinical application and
