孕前优生指导联合规范化孕期保健在高龄产妇中的应用

摘　　要：目的:探讨孕前优生指导联合规范化孕期保健在高龄产妇中的应用价值。方法:回顾性选取2019年1月-2021年4月武汉市江夏区妇幼保健计划生育服务中心的高龄产妇160例的临床资料。根据干预方法将其分为对照组和观察组,每组80例。对照组给予规范化孕期保健,观察组在对照组基础上进行孕前优生指导。比较两组妊娠6个月、预产期前1周、产后6 h情绪状态,干预后自我护理能力,妊娠期并发症及产后出血,分娩及新生儿结局。结果:观察组妊娠6个月、预产期前1周、产后6 h焦虑自评量表(SAS)及抑郁自评量表(SDS)评分均低于对照组。干预后观察组健康知识水平、自我概念、自我责任感、自我护理机能及总分均高于对照组(P<0.05)。观察组妊娠期高血压、糖尿病、贫血、胎膜早破、胎位异常及产后出血发生率均低于对照组(P<0.05)。观察组剖宫产、阴道助产率均低于对照组,阴道顺产率高于对照组(P<0.05)。结论:孕前优生指导联合规范化孕期保健可以减轻高龄产妇负性情绪,降低妊娠期及产后并发症及不良妊娠结局发生率,从而改善分娩结局,且可以提高产妇自我护理能力,应用效果明显。Objective:To explore the application value of pre-pregnancy eugenic guidance combined with standardized pregnancy care in elderly maternal.Method:From January 2019 to April 2021,the clinical data of 160 cases of elderly maternal in Family Planning Service Center of Maternal and Child Health Care in Jiangxia District of Wuhan City were retrospectively selected.They were divided into control group and observation group by the intervention method,80 cases in each group.The control group was given health care during pregnancy,and the observation group was given pre-pregnancy eugenic guidance on the basis of the control group.The emotional status at 6 months of pregnancy,1 week before the expected date of delivery and 6 h postpartum,self-care ability after intervention,pregnancy complications and postpartum hemorrhage,delivery and neonatal outcomes were compared between the two groups.Result:The scores of self rating anxiety scale(SAS)and self rating depression scale(SDS)in the observation group were lower than those in the control group at 6 months of pregnancy,1 week before the expected date of delivery and 6 h postpartum.After the intervention,the level of health knowledge,self-concept,self-responsibility,self-care function and total score of the observation group were higher than those of the control group(P<0.05).The incidences of hypertension,diabetes,anemia,premature rupture of membranes,abnormal fetal position in pregnancy and postpartum hemorrhage in the observation group were lower than those in the control group(P<0.05).The rate of cesarean section and vaginal assisted birth rate in the observation group were lower than those in the control group,and the rate of normal labor through vagina was higher than that in the control group(P<0.05).Conclusion:Pre-pregnancy eugenic guidance combined with standardized pregnancy care can alleviate the negative emotions of elderly maternal,reduce the incidence of pregnancy and postpartum complications and adverse pregnancy outcomes,thereby improving the outcome of childb
