当归六黄汤对气阴两虚型2型糖尿病患者动态葡萄糖图谱影响的临床研究

摘　　要：目的观察当归六黄汤对气阴两虚型2型糖尿病(T2DM)患者动态葡萄糖图谱(AGP)的影响。方法采用随机数字表法将62例气阴两虚型2型糖尿病患者分为对照组和观察组,每组31例。对照组予以盐酸二甲双胍缓释片、阿卡波糖片联合门冬胰岛素30注射液调控血糖;观察组在对照组降糖方案基础上加用当归六黄汤配方颗粒。两组疗程均为14天。治疗前后分别评估中医证候积分、胰岛素用量并分析受试者佩戴瞬感扫描式葡萄糖监测(FGM)设备提供的AGP报告相关指标:平均血糖水平(MBG)、血糖水平标准差(SDBG)、变异系数(CV)、预估糖化血红蛋白(eHbA1c)、平均血糖波动幅度(MAGE)、日间血糖平均绝对差(MODD)、葡萄糖高于目标范围时间(TAR)、葡萄糖在目标范围内时间(TIR)、葡萄糖低于目标范围时间(TBR)。结果62例患者中,其中60例完成试验。两组治疗后比较,观察组中医证候积分改善明显优于对照组(P<0.01),观察组胰岛素用量较对照组减少(P<0.05),AGP报告显示,观察组MBG、SDBG、CV、eHbA1c、MODD、MAGE、TAR、TBR与对照组相比均下降,其中eHbA1c、MODD、MAGE、TBR下降有统计学意义(P<0.05),TIR增加显著(P<0.01)。结论当归六黄汤可以增加气阴两虚型T2DM患者的目标范围内时间,减少低血糖发生率,改善日内、日间血糖波动水平及长期平均血糖水平,减轻临床症状。Objective To observe the effect of Danggui Liuhuang Decoction on the ambulatory glucose profile(AGP)of type 2 diabetes mellitus(T2DM)patients with deficiency type of Qi and Yin.Methods Using a random number table method,62 pa-tients with type 2 diabetes mellitus with deficiency type of Qi and Yin were divided into control group and observation group,31 patients in each group.The control group was given metformin hydrochloride sustained-release tablets,acarbose tablets com-bined with insulin aspart 30 injection to regulate blood glucose.The observation group was added with Danggui Liuhuang Decoc-tion Formula granules on the basis of the hypoglycemic scheme in the control group,both groups were treated for 14 days.Before and after treatment,evaluate the TCM syndrome scores and insulin dosage,and analyze the relevant indicators of the AGP report of the subjects wear the FGM equipment:the mean blood glucose level(MBG),standard deviation of blood glucose level(SD-BG),coefficient of variation(CV),estimated HbA1c(eHbA1c),mean amplitude of glycemic excursions(MAGE),means of daily differences(MODD),time above range(TAR),time in range(TIR),time below range(TBR).Results Among 62 pa-tients,60 patients completed the trial.The two groups after treatment,the improvement of TCM syndrome score in the observation group was significantly better than that in the control group(P<0.01),the dosage of insulin in the observation group was less than that in the control group(P<0.05).According to the AGP report:MBG,SDBG,CV,eHbA1c,MODD,MAGE,TAR,TBR in the observation group were lower than those in the control group.Among them,eHbA1c,MODD,MAGE and TBR de-crease was statistically significant(P<0.05).TIR Increase significantly(P<0.01).Conclusion Danggui Liuhuang decoction can increase the target time of T2DM patients with deficiency type of Qi and Yin,reduce the incidence of hypoglycemia,improve intra day and inter day blood glucose fluctuation level and long-term average blood glucose level,reduce clinical symptoms.
