摘  要:目的分析深圳市罗湖区呼吸道感染患儿外周血淋巴细胞亚群的水平变化,为该地区儿童呼吸道感染的临床诊断及治疗提供指导。方法选取2019年9月至2020年8月深圳市罗湖区人民医院收治的呼吸道感染患儿共736例为研究对象,将其中已明确病原学诊断的423例患儿作为感染组,同时选取同期20例体检健康儿童作为对照组。采用流式细胞术检测外周血淋巴细胞亚群。分析呼吸道感染患儿的年龄分布及不同季节呼吸道感染患儿的分布情况。比较感染组与对照组的淋巴细胞亚群水平,比较不同呼吸道感染疾病及不同病原体感染患儿的淋巴细胞亚群水平。结果在736例呼吸道感染患儿中,新生儿期患儿数量最少,学龄前期患儿数量最多;秋季就诊的患儿最多,其次依次为冬季、夏季,春季最少。感染组CD3^(+)CD4^(+)、CD3^(-)CD19^(+)、CD3^(+)CD4^(+)/CD3^(+)CD8^(+)水平均高于对照组,CD3^(+)CD8^(+)、CD3^(-)CD16^(+)CD56^(+)水平均低于对照组,差异有统计学意义(P<0.05)。上呼吸道感染组、支气管肺炎组、支气管炎组CD3^(+)CD8^(+)、CD3^(-)CD16^(+)CD56^(+)水平低于对照组,CD3^(-)CD19^(+)、CD3^(+)CD4^(+)/CD3^(+)CD8^(+)水平高于对照组,差异有统计学意义(P<0.05);支气管肺炎组、支气管炎组CD3^(+)CD4^(+)水平高于对照组与上呼吸道感染组,CD3^(-)CD16^(+)CD56^(+)水平低于上呼吸道感染组,CD3^(+)水平高于上呼吸道感染组,差异有统计学意义(P<0.05)。细菌组、支原体组、病毒组CD3^(+)CD8^(+)、CD3^(-)CD16^(+)CD56^(+)水平低于对照组,CD3^(-)CD19^(+)、CD3^(+)CD4^(+)/CD3^(+)CD8^(+)水平高于对照组,差异有统计学意义(P<0.05)。结论深圳市罗湖区儿童呼吸道感染的高发年龄段是学龄前期,高发季节是秋季。呼吸道感染患儿(已明确病原学诊断)与健康儿童的淋巴细胞亚群水平存在明显差异,且不同呼吸道感染疾病及不同病原体感染患儿的外周血淋巴细胞亚群水平Objective To analyze the changes of peripheral blood lymphocyte subsets in children with respiratory tract infection in Luohu District,Shenzhen,to provide guidance for the clinical diagnosis and treatment of respiratory tract infection in this area.Methods A total of 736 children with respiratory tract infection admitted to Shenzhen Luohu District People's Hospital from September 2019 to August 2020 were selected as the research objects.Among them,423 children with clear etiological diagnosis were selected as the infection group,and 20 healthy children underwent physical examination during the same period were selected as the control group.Peripheral blood lymphocyte subsets were detected by flow cytometry.The age distribution of children with respiratory tract infection and the distribution of children with respiratory tract infection in different seasons were analyzed.The lymphocyte subsets levels in the infection group and the control group were compared,and the lymphocyte subsets levels of children with different respiratory infection diseases and different pathogens were compared.Results Among the 736 children with respiratory tract infection,the number of children in the neonatal period was the smallest,and the number of children in the preschool age period was the largest.The most children were treated in autumn,followed by winter,summer,and the least in spring.The levels of CD3^(+)CD4^(+),CD3^(-)CD19^(+),CD3^(+)CD4^(+)/CD3^(+)CD8^(+)in the infection group were higher than those in the control group,and the levels of CD3^(+)CD8^(+),CD3^(-)CD16^(+)CD56^(+)were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of CD3^(+)CD8^(+),CD3^(-)CD16^(+)CD56^(+)in the upper respiratory tract infection group,bronchopneumonia group and bronchitis group were lower than those in the control group,and the levels of CD3^(-)CD19^(+),CD3^(+)CD4^(+)/CD3^(+)CD8^(+)were higher than those in the control group,the differences were statistically significant(P<0.05).The levels
