摘  要:目的:观察针刺运动区、足运感区配合体针对脑卒中患者平衡功能的影响。方法:将60例脑卒中后平衡功能障碍患者随机分为试验组30例和对照组30例。治疗组采用针刺配合常规康复训练,针刺方案为针刺运动区、足运感区配合体针。对照组采用和治疗组相同的单纯康复训练。观察两组治疗前后Berg平衡量表(Berg Balance Scale,BBS)评分、“起立-行走”计时测试(Timed Up and Go Test,TUGT)评分,并采用Prokin平衡评估系统(PK254)评估患者的平衡功能,包括对称性[X轴平均压力中心(Centre of Pressure,COP)]、COP运动椭圆面积及轨迹长度。结果:两组患者治疗3周后Berg平衡量表(Berg Balance Scale,BBS)评分、TUGT用时、X轴平均COP、运动椭圆面积及轨迹长度与同组治疗前比较,差异均具有统计学意义(P<0.01)。试验组治疗后BBS评分、TUGT用时、X轴平均COP、运动椭圆面积及轨迹长度与对照组比较,差异均具有统计学意义(P<0.01,P<0.05),且试验组疗效提高更显著。结论:针刺运动区、足运感区配合体针可有效提高脑卒中患者的动静态平衡功能及行走能力,减少患者机体摆动幅度,改善重心对称性及稳定性,从而降低跌倒的风险。Objective:To observe the effect of scalp acupuncture(inserting at the motor area and foot motor sensory area)and body acupuncture on the balance function of stroke patients.Methods:Sixty patients with balance dysfunction after stroke were randomly divided into a treatment group of 30 cases and a control group of 30 cases.In the treatment group,acupuncture combined with conventional rehabilitation training were applied,and the selection of acupuncture points included acupuncture at the motor area and foot motor sensory area(Jiaoshi scalp acupuncture)and points of body acupuncture.The control group used the same simple rehabilitation training as the treatment group.Before and after the treatment,the Berg Balance Scale(BBS)scores and the“Stand-Walk”Timing Test(TUGT)scores of the two groups were observed,and the Prokin balance assessment system(PK254)was applied to evaluate the patient’s balance function,including symmetry[X-axis average centre of pressure(COP)],the ellipse area and track length of the center of pressure(COP).Results:After 3 weeks of treatment,BBS score,TUGT time,X-axis average COP,motion ellipse area and trajectory length of the two groups were significantly different from those before treatment(P<0.01).After treatment,the berg balance scale(BBS)score,TUGT time,X-axis average COP,motion ellipse area and trajectory length of the treatment group were significantly different from those of the control group(P<0.01,P<0.05),and the effect in the treatment group was significantly improved compared with that of the control group.Conclusion:Acupuncture in the motor area and foot motor sensory area combined with body acupuncture can effectively improve the dynamic and static balance function and walking ability of stroke patients,reduce the body swing amplitude,and improve the symmetry and stability of the center of gravity,so as to reduce the risk of falls.
