断指再植术后应用补气活血与温经通脉法的临床效果观察

摘　　要：目的:研究断指再植术后应用补气活血与温经通脉法的临床效果。方法:将2019年1月-2020年12月收治的断指再植术后患者80例随机分为研究组与对照组,各40例,对照组给予常规三抗(抗感染、抗痉挛、抗凝)及支持治疗,研究组在对照组基础上联合补气活血与温经通脉法进行治疗。观察比较两组治疗后临床疗效,记录比较两组治疗第2周、第4周时患指红肿评分,记录两组治疗前后全血及血浆黏度、纤维蛋白原水平,观察两组治疗后血管危象发生率、断指成活率。结果:研究组总有效率明显高于对照组(P<0.05)。治疗第2周、第4周时患指红肿评分明显低于对照组(P<0.05),治疗后全血及血浆黏度、纤维蛋白原水平明显低于对照组(P<0.05);研究组治疗后未发生血管危象,对照组发生4例血管危象,观察组血管危象发生率明显低于对照组(P<0.05);研究组断指成活率明显高于对照组(P<0.05)。结论:在临床常规治疗的基础上联合应用补气活血与温经通脉法可提高断指再植术后的临床效果,避免凝血功能障碍等不良事件的发生,从而缓解患指肿胀等症状,且有利于降低血管危象发生率,提高断指成活率。Objective:To study the clinical effect of the method of invigorating Qi and promoting blood circulation and warming meridians and clearing arteries after replantation of severed finger.Methods:A total of 80 patients with severed finger replantation from January 2019 to December 2020 were randomly divided into study group and control group,with 40 patients in each group.The control group was given conventional triple anti-infection(anti-spasticity,anti-coagulation)and supporting treatment,while the study group was treated with the method of invigorating qi and promoting blood circulation and warm meridian and passing pulse on the basis of the control group.Clinical efficacy after treatment was observed and compared between the two groups.The scores of finger redness and swelling at the 2nd and 4th week of treatment were recorded between the two groups.The whole blood and plasma viscosity and fibrinogen levels before and after treatment were recorded between the two groups.Results:The total effective rate in the study group was significantly higher than the control group(P<0.05).At the second and fourth week of treatment,the redness and swelling scores of the affected fingers were significantly lower than the control group(P<0.05),and the whole blood and plasma viscosity and fibrinogen levels were significantly lower than the control group(P<0.05).No vascular crisis occurred in the study group after treatment,but 4 cases in the control group.The incidence of vascular crisis in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the survival rate of amputated finger in the study group was significantly higher than that in the control group(P<0.05).Conclusion:On the basis of routine clinical treatment,the combined application of invigorating Qi and promoting blood circulation with warming meridian and passing arteries can improve the clinical effect after replantation of severed finger,avoid the occurrence of adverse events such as coagulation dysfunction,relieve the s
