经筋推拿联合肌内效贴治疗乳腺癌术后上肢淋巴水肿临床观察

摘　　要：目的:观察经筋推拿联合肌内效贴治疗乳腺癌术后上肢淋巴水肿的临床疗效。方法:将168例乳腺癌术后上肢淋巴水肿患者采用随机数字分表法分为常规治疗组、经筋推拿组、肌内效贴组和经筋推拿联合肌内效贴组(联合组),每组42例。常规治疗组患者采用常规治疗;经筋推拿组患者在常规治疗基础上加用经筋推拿;肌内效贴组患者在常规治疗基础上加用肌内效贴布贴扎治疗;联合组患者在常规治疗基础上进行经筋推拿联合肌内效贴治疗。比较4组患者治疗前后上臂臂围差值变化、临床疗效、治疗前后上肢淋巴流量变化和上肢功能评定量表(DASH)评分变化。结果:治疗后4组患者上臂臂围差值和DASH评分均低于治疗前(P<0.05),淋巴流量均高于治疗前(P<0.05);治疗后经筋推拿组、肌内效贴组和联合组患者上臂臂围差值和DASH评分均低于常规治疗组(P<0.05),淋巴流量均高于常规治疗组(P<0.05);治疗后联合组患者上臂臂围差值和DASH评分均低于经筋推拿组和肌内效贴组(P<0.05),淋巴流量高于经筋推拿组和肌内效贴组(P<0.05);治疗后经筋推拿组患者上臂臂围差值,淋巴流量和DASH评分与肌内效贴组患者比较,差异均无统计学意义(P>0.05);4组临床疗效等级和总有效率比较差异均有统计学意义(P<0.05),联合组总有效率高于常规治疗组(P<0.01)。结论:经筋推拿和肌内效贴均能够减轻患者乳腺癌术后上肢淋巴水肿症状,经筋推拿联合肌内效贴治疗效果最佳,且经筋推拿联合肌内效贴能够显著缓解患者淋巴水肿症状,提高临床疗效,改善患者淋巴回流障碍,提升患者上肢肢体功能。Objective:To observe the effect of channel sinew tuina combined with kinesiology tape on lymphedema of upper limb after breast cancer surgery.Methods:One hundred and sixty eight patients with lymphedema of upper limbs after breast cancer surgery were divided into routine treatment group,channel sinew tuina group,kinesiology tape group and channel tuina combined with kinesiology tape group(combination group) accord to random number table,42 patients in each group. The patients in the conventional treatment group were treated with routine method. The patients in the channel sinew tuina group were treated with channel sinew tuina besides routine treatment. The patients in the kinesiology tape group were treated with kinesiology tape besides routine treatment. The patients in the combined group were treated with channel sinew tuina combined with kinesiology tape besides routine treatment. The changes of arm circumference difference before and after treatment,clinical efficacy,lymph flow before and after treatment and disability of the arm,shoulder and hand questionnaire(DASH) scores before and after treatment were compared among the four groups. Results:After treatment,arm circumference difference and DASH scores of the four groups were lower than those before treatment(P<0.05),and lymph flow was higher than that before treatment(P<0.05). After treatment,the arm circumference difference and DASH scores of the channel sinew tuina group,kinesiology tape group and combination group were lower than those of the routine treatment group(P<0.05),and the lymph flow was higher than that of the routine treatment group(P<0.05). After treatment,the arm circumference difference and DASH scores in the combined group were lower than those in the channel sinew tuina group and kinesiology tape group(P<0.05),and the lymph flow was higher than that in the channel sinew tuina group and kinesiology tape group(P<0.05). After treatment,there were no significant difference in arm circumference,lymph flow and DASH scores between the channel
