摘 要:目的观察胰岛素泵(CS2)联合二甲双胍治疗对2型糖尿病患者人工膝关节置换术后围手术期并发症的影响。方法选取我院2016年6月至2021年2月采用人工膝关节表面置换术治疗伴有2型糖尿病骨关节炎患者78例。随机分为胰岛素泵组(A组)和胰岛素泵联合二甲双胍组(B组),其中A组38例,B组40例。比较两组治疗前后空腹血糖(FBG)、餐后2 h血糖(2hBG)及其达标时间、胰岛素用量和对术后4周围术期并发症的影响。结果A、B组空腹血糖、餐后2 h血糖达标时间均有统计学意义(P<0.05);胰岛素用量分别为(29.80±5.02)U·d^(-1)和(19.16±5.13)U·d^(-1),差异有统计学意义(P<0.01)。A组深静脉血栓形成(DVT)1例、浅表手术部位感染(SSI)1例。B组下呼吸道感染(LRTI)1例。结论胰岛素泵联合二甲双胍比单独使用胰岛素泵治疗能缩短人工膝关节表面置换术前围手术期时间,胰岛素泵和胰岛素泵联合二甲双胍治疗均有效减少人工膝关节表面置换术后急性围手术期并发症。Objective To observe the effect of Continuous Subcutaneous Insulin Infusion(CS2)combined with Metformin on complications after total knee arthroplasty(2)in patients with type 2 diabetes mellitus.Methods From June 2016 to February 2021,78 patients with osteoarthritis of type 2 diabetes mellitus were treated with total knee arthroplasty.They were randomly divided into Continuous Subcutaneous Insulin Infusion,CS2(group A)and CS2 combined with Metformin(group B),including 38 cases in group A and 40 cases in group B.The recovering time of FBG,2hBG and BG before and after treatment and their effects on acute perioperative complications 4 weeks after operation were compared between the two groups.Results The reaching times of FBG and 2hBG in group A and B had statistically significant(P<0.05);The dosage of insulin was(29.80±5.02)and(19.16±5.13)U·d^(-1),respectively(P<0.01),the difference was statistically significant(P<0.01).In group A,there were 1 case of deep venous thrombosis(DVT)and 1 case of superficial surgical site infection(SSI).There was 1 case of lower respiratory tract infection(LRTI)in group B.Conclusion CS2 combined with Metformin can shorten the perioperative time before knee arthroplasty than CS2 alone.Both CS2 and CS2 combined with Metformin can effectively reduce the acute perioperative complications after knee arthroplasty.
