摘  要:背景:临床以及前期研究发现热敏灸治疗银屑病效果良好,但具体机制尚未明确。目的:探讨热敏灸对银屑病样大鼠的影响及其机制。方法:选取60只8-10周龄健康SD大鼠,随机分为空白组15只、模型组15只、治疗组30只。除空白组外,其余各组大鼠背部和手少阳三焦经所循行的皮肤进行银屑病皮损造模,建模成功后,治疗组于每日同一时间对手少阳三焦经外关穴进行热敏灸治疗,每次40 min,连续7 d,根据每日艾灸过程中尾温的变化情况分为热敏灸组15只与艾灸组15只。对各组大鼠进行银屑病皮损面积及严重程度(PASI)评分,采用Western blot检测大鼠外关穴皮肤和筋膜中JAK3、STAT3、p-JAK3以及p-STAT3的蛋白表达。结果与结论:(1)艾灸组、热敏灸组银屑病皮损面积及严重程度评分均低于模型组(P<0.05),热敏灸组银屑病皮损面积及严重程度评分低于艾灸组,但差异无显著性意义(P>0.05);(2)与模型组相比,艾灸组大鼠外关穴处筋膜中p-JAK3、p-STAT3蛋白表达显著降低(P<0.05),艾灸组大鼠外关穴处皮肤中p-JAK3蛋白表达显著降低(P<0.05);(3)与模型组相比,热敏灸组大鼠皮肤和筋膜中JAK3、STAT3、p-JAK3、p-STAT3蛋白表达均显著降低(P<0.05);(4)与艾灸组相比,热敏灸组外关穴筋膜中STAT3蛋白表达显著降低(P<0.05);(5)结果表明,艾灸和热敏灸可改善银屑病样大鼠皮损病变,抑制皮损炎症,其中热敏灸的治疗效果在一定程度上优于艾灸,其机制可能与抑制STAT3通路信号转导有关。BACKGROUND:Heat-sensitive moxibustion is confirmed to be effective in the treatment of psoriasis,but the specific mechanism is still unclear.OBJECTIVE:To explore the effect of heat-sensitive moxibustion on psoriasis rats and its mechanism.METHODS:Sixty healthy Sprague-Dawley rats aged 8-10 weeks were randomly divided into blank control group(n=15),model group(n=15),and treatment group(n=30).Except for the blank group,animal models of psoriatic lesions were made on the back and the skin along the Sonjioo meridian of hand-Shooyong.After successful modeling,the treatment group was treated with heat-sensitive moxibustion at Woiguon point of the Sanjiao meridian of ha nd-Shaoyang at the same time every day,40 minutes once,for 7 continuous days.The treatment group was subdivided into heat-sensitive moxibustion group(n=15)and moxibustion group(n=15)according to the changes of tail temperature in the daily moxibustion process.After treatment,the rats in each group were sco red by the psoriasis area and severity index(PASI),and after treatment,the protein expressions of JAK3,STAT3,p-JAK3,and p-STAT3 in the skin and fascia of Waiguan point were detected by western blot assay.RESULTS AND CONCLUSION:The PASI scores in the moxibustion group and heat-sensitive moxibustion group were lower than that in the model group,while the PASI score in the heat-sensitive moxibustion group was lower than that in the moxibustion group and there was no significant difference between groups.Compared with the model group,the protein expressions of p-JAK3 and p-STAT3 in the fascia of Woiguon point were significantly decreased(P<0.05),and the protein expression of p-JAK3 in the skin of Woiguan point was also significantly reduced in the moxibustion group(P<0.05).Compared with the model gro up,the protein expressions of JAK3,STAT3,p-JAK3,and p-STAT in the fascia and skin of Waiguon point were significantly decreased in the heat-sensitive moxibustion group(P<0.05).Compared with the moxibustion group,the protein expression of STAT3 in the fascia of
