自拟中药联合穴位贴治疗老年肺脾肾虚型哮喘的临床疗效

摘　　要：目的观察自拟中药联合穴位贴治疗老年肺脾肾虚型哮喘患者的临床疗效。方法选取240例我院2017年1月至2019年1月接诊的老年非急性期肺脾肾虚型哮喘患者,随机分为观察组(予自拟中药联合穴位贴疗法)与对照组(予糖皮质激素治疗)。治疗1个月后观察疗效,检测治疗前后两组患者血清中ILC2、IgE、IL-4水平,治疗后随访24个月,检测患者肺功能、血气指标。结果两组患者ILC2、IgE、IL-4治疗后均低于治疗前(P<0.05),以观察组更为明显(P<0.05),两组患者的肺功能指标和血气指标PaO_(2)、pH治疗后高于治疗前(P<0.05),以观察组更明显(P<0.05),血气指标PaCO_(2)治疗后低于治疗前(P<0.05),观察组降低更明显(P<0.05);两组患者哮喘症状评分治疗后低于治疗前(P<0.05),以观察组更明显(P<0.05);观察组总有效率高于对照组(P<0.05)。结论自拟中药联合穴位贴治疗老年肺脾肾虚型哮喘,可提降低ILC2、IgE、IL-4的表达,改善肺功能指标,较传统糖皮质激素疗法效果更好。Objective To observe the clinical efficacy of self-made traditional Chinese medicine combined with Acupoint Plaster in the treatment of elderly patients with asthma of lung,spleen and kidney deficiency.Methods 240 elderly patients with non acute lung,spleen and kidney deficiency asthma treated in our hospital from January 2017 to January 2019 are selected,which are divided into observation group(treated with selfmade traditional Chinese medicine combined with acupoint sticking therapy)and control group(treated with glucocorticoid).The curative effect is observed after 1 month of treatment.The levels of serum ILC2,IgE and IL-4 in the two groups are detected before and after treatment.The patients are followed up for 24 months to detect the indexes of lung function and blood gas.Results After treatment,ILC2,IgE and IL-4 in the two groups are lower than those before treatment(P<0.05),and the observation group is more obvious(P<0.05).The indexes of pulmonary function and blood gas PaO_(2),pH of patients in two groups are higher than those before treatment(P<0.05),and the observation group is more obvious(P<0.05).Blood gas index PaCO_(2) is lower than before treatment(P<0.05),and the observation group is more obvious(P<0.05).The asthma symptom score of the two groups after treatment is lower than that before treatment(P<0.05),and the observation group is more obvious(P<0.05).The total effective rate of the observation group is higher than that of the control group(P<0.05).Conclusion The self-made traditional Chinese medicine combined with Acupoint Plaster can improve and reduce ILC2 and IgE in the treatment of Senile Asthma with deficiency of lung,spleen and kidney.The expression of IL-4 can improve the indexes of lung function,which is better than traditional glucocorticoid therapy.
