基于护士主导的自我管理结合运动康复护理对慢性心力衰竭患者自我管理及心功能的影响

摘　　要：目的:探讨基于护士主导的自我管理结合运动康复护理对慢性心力衰竭(CHF)患者自我管理及心功能的影响。方法:纳入我院2019年6月至2020年11月收治的146例CHF患者,按随机数字表法分为对照组73例和观察组73例。观察组在对照组常规干预基础上实施基于护士主导的自我管理结合运动康复护理。比较两组心功能、自我管理能力及血浆脑钠肽(BNP)、醛缩酶(ALD)水平。结果:比较两组护理前心功能、自我管理能力及BNP、ALD水平,无统计学差异(P>0.05);与对照组相比,观察组护理后左心射血分数(LVEF)、6 min步行距离(6MWT)较高,N末端B型利钠肽(NT-poBNP)较低,自我管理能力评分、BNP、ALD水平均较低,有统计学差异(P<0.05)。结论:基于护士主导的自我管理结合运动康复护理能够增强CHF患者心功能,提高自我管理能力,促进病情改善。Objective:To explore the effect of nurse-led self-management combined with exercise rehabilitation nursing on self management and cardiac function of patients with chronic heart failure(CHF).Methods:A total of 146 CHF patients admitted to our hospital from June 2019 to November 2020 were enrolled and divided into 73 control groups and 73 observation groups according to the random number table method.On the basis of routine intervention in the control group,the observation group implemented nurse-led self-management combined with sports rehabilitation nursing.The cardiac function,self-management ability and plasma brain natri⁃uretic peptide(BNP)and aldolase(ALD)levels were compared between the two groups.Results:The cardiac function,self-manage⁃ment ability,BNP and ALD levels of the two groups before nursing were compared,and there was no statistical difference(P>0.05).Compared with the control group,the observation group left ventricular ejection fraction(LVEF)and 6min walking distance after nurs⁃ing(6MWT)is higher,N-terminal B-type natriuretic peptide(NT-poBNP)is lower,self-management ability score,BNP,and ALD lev⁃els are lower,with statistical differences(P<0.05).Conclusion:Based on nurseled self-management combined with exercise reha⁃bilitation nursing can enhance the heart function of CHF patients,improve self-management ability,and promote the improvement of the condition.
