摘　　要：目的:比较颗粒包埋法与匀浆注射法两种甲状旁腺自体移植术在甲状旁腺全切除治疗继发性甲状旁腺功能亢进中的应用效果。方法:选取本院SHPT患者14例,随机分为2组,所有患者均行甲状旁腺全切除,7例采取颗粒包埋法移植,7例采取匀浆注射法移植。对比2组患者术前1天、术后1天、术后1月的PTH水平、血钙水平以及生存质量评分情况。结果:2组患者术前1天、术后1天、术后1月的PTH及血清钙相比较,差异无统计学意义(P>0.05)。2组患者术前1天、术后1月的生存质量评分,差异无统计学意义(P>0.05)。所有患者的术前、术后生存质量评分,差异有统计学意义(P<0.05)。结论:联合腺体自体移植的甲状旁腺全切除治疗继发性甲状旁腺功能亢进的效果良好,颗粒包埋法与匀浆注射法甲状旁腺自体移植均安全有效且效果相近,且后者操作更加简易。Objective:To compare the effect of two kinds of autologous transplantation of parathyroid glands in the treatment of secondary hyperparathyroidism by Total parathyroidectomy. Methods:Fourteen patients with SHPT were randomly divided into 2 groups. Total parathyroidectomy was performed in all patients. 7 patients were implanted by embedding granules and 7 by homogenate injection. The PTH level,serum calcium level and quality of life of the two groups were compared 1 day before operation,1 day after operation and 1 month after operation. Results:There was no significant difference in PTH and serum calcium between two groups at 1 day before operation,1 day after operation and1 month after operation. There was no significant difference between the two groups in quality of life scores 1 day before operation and 1 month after operation(P>0.05). The scores of preoperative and postoperative quality of life in all patients were significantly different(P<0.05). Conclusion:Total parathyroidectomy combined with auto-transplantation has a good effect in the treatment of secondary hyperparathyroidism. The particle embedding method is safe and effective,and the effect is similar to that of auto-transplantation with homogenate injection,the latter is easier to operate.
