星蒌承气汤治疗中风病痰热腑实证疗效及对患者生物学指标和血浆内毒素脂多糖的影响

摘　　要：目的:探讨星蒌承气汤对中风病痰热腑实证患者生物学指标及血浆内毒素脂多糖(LPS)的影响,为其临床应用提供参考。方法:选择96例中风病痰热腑实证患者,随机分为对照组和观察组各48例。两组患者均给予西医常规治疗,观察组在此基础上给予星蒌承气汤联合治疗,均治疗14 d。比较两组患者治疗前后神经功能缺损评分(NIHSS)、巴氏指数(Barthel)评分、中风症候要素积分差异,分析两组患者治疗前后血清生物学指标及血浆内毒素脂多糖(LPS)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平变化,比较两组疗效。结果:观察组治疗总有效率高于对照组(P<0.05)。治疗后,两组血清核苷二磷酸激酶A水平、血浆LPS、TNF-α、IL-6水平、NIHSS评分及内风、内火、痰湿、血瘀、气虚、阴虚等中风证候要素积分均明显降低,血清人帕金森蛋白7水平及Barthel评分均明显升高,且观察组上述指标改善更为明显(均P<0.05)。结论:星蒌承气汤可减轻中风病痰热腑实证患者炎性反应,改善其生物学指标及神经功能。Objective:To explore the effect of Xinglou Chengqi decoction on the biological indicators and plasma LPS of patients with phlegm-heat fu-organ syndrome in stroke disease,and to provide references for clinical diagnosis and treatment.Methods:A total of 96 patients with phlegm-heat and phlegm-heat syndrome were selected as the research objects.They were randomly divided into control group(n=48)and observation group(n=48).Patients in both groups were treated with conventional western medicine,and the observation group was treated with Xinglou Chengqi decoction on this basis.Both groups were treated for 14 days.The differences in neurological deficit score(NIHSS),Barthel index(Barthel)score,and stroke syndrome element scores were compared between the two groups of patients before and after treatment.Analyze the changes in serum biological indicators and plasma levels of endotoxin lipopolysaccharide(LPS),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)before and after treatment in the two groups of patients.Observe the difference in clinical efficacy between the two groups of patients after treatment.Results:The clinical efficacy of the observation group was significantly higher than that of the control group(P<0.05).Compared with before treatment,the level of serum NDKA,the level of plasma LPS,TNF-α,IL-6,NIHSS score and the score of stroke syndrome factors such as internal wind,internal fire,phlegm-dampness,blood stasis,Qi deficiency and Yin deficiency were significantly decreased in two groups after treatment.Serum PARK7 level and Barthel score were significantly increased,and the improvement of above indicators was more significant in the observation group(all P<0.05).Conclusion:Xinglou Chengqi decoction can reduce the inflammatory reaction of stroke patients with Phlegm-heat fu-organ syndrome,improve their biological indexes and neurological function.
