中医药治疗糖尿病性骨质疏松症从“脾肾论治”机制

摘　　要：糖尿病性骨质疏松症(DOP)是糖尿病(DM)常见并发症之一,严重影响DM患者的健康及生活质量。现代医学目前尚无行之有效的治疗方案。中医药治疗DOP方面具有独特优势,但中医在治疗DOP时多从“肝肾”论治,忽视了“脾”的重要作用。肾主骨生髓为先天之本,脾主肌肉充养四肢为后天之本,先天与后天相互资生、互惠互利,维持人体肌肉骨骼系统正常生理功能的发挥;两者病理上亦相互影响,互为因果,先天不足致后天失养,脾气虚弱,脾虚则肌肉削,肉削则骨枯。故从“脾肾论治”DOP亦尤为重要。现从“脾肾论治”DOP出发,结合现代中医药研究基础,探讨中医药从“脾肾论治”DOP的相关机制及必要性,为中医药防治DOP提供新思路。Diabetic osteoporosis(DOP)is one of the common complications of diabetes mellitus(DM),which seriously affects the health and quality of life of DM patients.Currently,there is no effective treatment plan in modern medicine.Traditional Chinese medicine has unique advantages in the treatment of DOP,but in the treatment of DOP,traditional Chinese medicine mostly focuses on the“liver and kidney”,ignoring the important role of the“spleen”.Kidney governs bone and marrow,which is innate,and spleen governs muscles and nourishes limbs,which is acquired.Innate and acquired are mutually supportive and mutually beneficial,and maintain the normal physiological functions of the human musculoskeletal system.Mutual cause and effect,congenital deficiency leads to acquired dystrophy,spleen deficiency,the muscles are cut,and the bones are withered.Therefore,from the“spleen and kidney treatment”,DOP is also particularly important.This article starts from the“spleen and kidney treatment”DOP,combined with the research foundation of modern Chinese medicine,to discuss the related mechanism and necessity of traditional Chinese medicine“spleen and kidney treatment”DOP,and provide new ideas for the prevention and treatment of DOP with traditional Chinese medicine.
