“从督论治” 针刺治疗颈椎病合并高血压病临床研究

摘　　要：目的:观察针刺“从督论治”颈椎病合并高血压患者的临床疗效。方法:选取颈椎病合并高血压患者60例,按照随机数字表法分为针刺组和对照组,对照组采用常规西医对症降压治疗,针刺组在对照组基础上采用针刺“从督论治”取穴治疗,两组均治疗4周。观察两组患者治疗前后血压、颈椎病躯体症状及血管内皮功能改善情况。结果:两组患者治疗前收缩压(SBP)、舒张压(DBP)、中医证候积分、血清肿瘤坏死因子(TNF-α)、内皮素-1(ET-1)、一氧化氮(NO)比较差异无统计学意义(P>0.05)。治疗后两组患者SBP、DBP均有不同程度下降,针刺组优于对照组(P<0.05);并且针刺组在颈椎病躯体症状改善起效时间上更短(P<0.05),对患者颈椎功能改善作用更显著(P<0.05)。治疗后两组血清TNF-α、ET-1都较前下降(P<0.05),NO较前升高(P<0.05),针刺组TNF-α、ET-1较对照组更低(P<0.05),NO高于对照组(P<0.05)。两组治疗过程中未出现严重不良反应,针刺组治疗效果较对照组更好(P<0.05)。结论:针刺“从督论治”颈椎病合并高血压患者具有较好的降压效果,并且能够降低炎症反应,改善血管内皮功能,安全性更高。Objective:To investigate the clinical efficacy of acupuncture from dumeridian in hypertensive patients complicated with cervical spondylosis.Methods:A total of 60 patients with hypertension complicated with cervical spondylosis and divided into acupuncture group and control group according to random number method.The control group received conventional medicine treatment,and the acupuncture group was treated with acupuncture from dumeridian based on the control group.Both groups were treated for 4 weeks.Blood pressure,somatic symptoms of cervical spondylosis and improvement of vascular endothelial function in 2 groups were observed before and after treatment.Results:Before treatment,there were no significant differences between 2 groups in systolic blood pressure(SBP),diastolic blood pressure(DBP),TCM syndrome score,serum tumor necrosis factor(TNF-α),endothelin-1(ET-1)and nitric oxide(NO)(P>0.05).After treatment,both of SBP and DBP in 2 groups decreased to different degrees,and acupuncture group was better than control group(P<0.05).Moreover,acupuncture group had shorter time in improving the physical symptoms of cervical spondylosis(P<0.05),and more significant effect on the improvement of cervical vertebra function(P<0.05).After treatment,serum TNF-αand ET-1 in both groups were decreased(P<0.05),and NO was increased(P<0.05).In acupuncture group,serum TNF-αand ET-1 were lower than that in control group(P<0.05),and NO was higher than that in control group(P<0.05).There were no serious adverse reactions in the two groups,and the therapeutic effect of acupuncture group was better than the control group(P<0.05).Conclusion:Acupuncture from dumeridian has better antihypertensive effect,and can reduce inflammatory response,improve vascular endothelial function,with higher safety.
