摘　　要：目的:分析V-Y推进皮瓣在面部皮肤肿瘤患者术后缺损修复中的应用效果。方法:选取2016年1月-2020年2月笔者医院收治的41例面部皮肤肿瘤患者作为研究对象,依据患者缺损部位、形状、面积给予修复方案。其中,经典V-Y推进皮瓣10例,穿支V-Y推进皮瓣10例,改良V-Y推进皮瓣21例。结果:41例患者中,39例患者一期愈合,1例出现皮瓣边缘坏死,经局部修剪换药后愈合;1例出现局部切口感染,经短期换药后愈合。术后口角易位1例、睑外翻1例。随访10~37个月,40例患者面部功能恢复较好,1例患者局部复发,再次行手术切除并放疗。结论:通过V-Y推进皮瓣技术修复面部皮肤肿瘤切除术后缺损,可降低术后并发症发生率,且美观度较好。Objective To analyze the effect of V-Y advanced skin flap in postoperative defect repair of patients with facial skin tumor. Methods A total of 41 patients with facial skin tumors admitted to our hospital from January 2016 to February 2020were selected as the research objects, and the repair plan was given according to the defect location, shape and area of the patients. Among them, there were 10 cases of classical subcutaneous pedicle V-Y propelled skin flap, 10 cases of perforator branch V-Y propelled skin flap, and 21 cases of modified V-Y propelled skin flap. Results Among the 41 patients, 39 patients had primary healing, and one patient had marginal necrosis of skin flap, which healed after local pruning and dressing change.Local incision infection occurred in one patient, which healed after short-term dressing change. There was one patient of angular displacement and one patient of ectropion. After follow-up of 10 to 37 months, 40 patients had good facial function recovery, one patient had local recurrence, and underwent surgical resection and radiotherapy again. Conclusion Using V-Y advancement skin flap technology to repair the defect after facial skin tumor resection can reduce the incidence of postoperative complications and have good aesthetics.
