山东省公立医院助产适宜技术应用的分区域调查及经济效益分析

摘　　要：目的调查山东省各区域公立医院助产适宜技术的应用情况,并分析其与经济效益的相关性。方法采用便利抽样法对山东省162所公立医院进行问卷调查,回收154份有效问卷。结果自然分娩率为50.5%(40%,60%),会阴侧切率为20%(15%,34.25%)。不同级别医院会阴侧切率差异有统计学意义(P=0.038)。66.2%的医院有增加助产士的需求;医院实施了各种助产适宜技术:导乐(86.2%)、自由体位分娩(76.0%)、心理支持(79.9%)、拉玛泽呼吸(64.9%)、硬膜外麻醉(60.4%)、水中分娩(2.6%);自然分娩量、住院费用与当地人均国内生产总值(GDP)之间的相关性具有统计学意义(P=0.048和P=0.004)。结论根据山东省的研究成果,有必要进一步增加助产士的数量和推广助产技术的应用。在一定程度上,运用助产技术可以有效地促进当地经济的发展。Objective To investigate the application of appropriate technologies of midwifery in Shandong governmental hospitals in various regions,and analyze its correlation with economic benefits.Methods A questionnaire was conducted to investigate by convenient sampling in 162 Shandong governmental hospitals,and 154 valid questionnaires were collected.Results The rate of natural delivery was 50.5%(40%,60%),and the rate of perineal lateral resection was 20%(15%,34.25%).Perineal lateral resection rate′s differences in different levels of hospitals were statistically significant(P=0.038).66.2%of hospitals have an increased need for midwives;The hospital carried out a variety of appropriate technologies of midwifery:guided delivery(86.2%),free position delivery(76.0%),psychological support(79.9%),lamaze breathing(64.9%),epidural anesthesia(60.4%),and water delivery(2.6%);The correlation between the number of natural births,the cost of guiding and midwifery in hospitals and local GDP per capita was statistically significant(P=0.048 and P=0.004).Conclusion According to the research results in Shandong,it is necessary to further increase the number of midwives and promote the use of appropriate technologies of midwifery.To a certain extent,appropriate technologies of midwifery can effectively promote the development of the local economy.
