摘　　要：目的 研究乳腺单纯可疑恶性钙化病变的MRI表现。方法 分析2017年10月~2021年3月于复旦大学附属中山医院青浦分院行MRI检查,且乳腺X线摄影评价为单纯可疑恶性钙化的病变,经手术或穿刺活检得到病理证实的患者中,选取53例均表现为非肿块样强化者,以病理结果为金标准,分析非肿块样强化的分布、内部强化特征、时间-信号强度曲线、表观扩散系数(apparent diffusion coefficient, ADC)与良恶性关系。结果 恶性病变中以叶段样及线样强化分布者居多;内部强化以簇环状、集丛状者为多;时间-信号强度曲线(TIC)以Ⅱ型、Ⅲ型者多见,且差异有统计学意义(P<0.05);恶性病变ADC无明显减低(P>0.05)。结论 乳腺MRI检查可弥补乳腺X线摄影的不足,对良恶性钙化的鉴别有一定价值。Objective To study the MRI findings of simple suspicious malignant calcification of breast. Methods From October 2017 to March 2021, 53 patients who underwent MRI examination in QingPu Branch of Zhongshan Hospital Affiliated to Fudan University and whose breast lesions were purely suspicious of presenting malignant calcification on breast X-ray photography and confirmed by surgery or biopsy were selected. The distribution, internal enhancement characteristics, time signal intensity curve, time signal intensity curve of non mass like enhancement, and apparent diffusion coefficient(ADC) difference between benign and malignant leison were analyzed with pathological results as the gold standard. Results The majority of malignant lesions were lobular and linear enhancement, the majority of internal enhancement were cluster ring and cluster, and the most common types of TIC were type Ⅱ and type Ⅲ, and the difference was statistically significant(P<0.05);There was no significant decrease in ADC in malignant lesions(P>0.05). Conclusion Breast MRI examination is able to make up for the lack of mammography, and has a certain value in the differential diagnosis of benign and malignant calcification.
