摘　　要：目的观察红元胶囊联合多模式镇痛对老年股骨粗隆间骨折患者围手术期的镇痛效果。方法将78例老年股骨粗隆间骨折患者按照随机数字表法分为2组,对照组36例采用多模式镇痛方案,治疗组42例采用红元胶囊联合多模式镇痛方案。比较2组术前、术后12、24、48、72 h和7 d静息状态下疼痛视觉模拟评分(VAS),术后12、24、48 h静脉自控镇痛泵(PCIA)按压人次,术后12、24、48、72 h不良反应发生率,术后第7天髋关节前屈、外展活动度。结果2组术后12、24、48、72 h和7 d疼痛VAS均较本组术前降低(P<0.05),且治疗组术后12、24、48、72 h疼痛VAS均低于对照组同期(P<0.05)。治疗组术后12、24、48 h PCIA按压人次均少于对照组同期(P<0.05),术后12、24、48、72 h不良反应发生率均低于对照组同期(P<0.05),术后第7天髋关节前屈、外展活动度均大于对照组(P<0.05)。结论红元胶囊联合多模式镇痛方案对老年股骨粗隆间骨折围手术期患者镇痛效果良好,可减少阿片类药物用量及药物不良反应,促进髋关节功能恢复。Objective To observe the efficacy of Hongyuan capsule combined with multimodal analgesia for perioperative analgesic in elderly patients with intertrochanteric fracture of hip.Methods Totally 78 elderly patients with intertrochanteric fracture of hip were randomly assigned to treatment group(n=42)and control group(n=36).All patients were managed by multimodal analgesia,and those in treatment group were additionally treated with Hongyuan capsule.The aim was to compare visual analogue scale(VAS)in resting state at preoperation,postoperative hours 12,24,48,72 and postoperative day 7;patient-controlled intravenous analgesia(PCIA)at postoperative hours 12,24,48;adverse reactions(AR)rate at postoperative hours 12,24,48,72;hip-flexion range of motion(ROM),hip joint ROM at postoperative day 7.Results Compared with before surgery,VAS at postoperative hours 12,24,48,72 and postoperative day 7 in groups was decreased(P<0.05);VAS at postoperative hours 12,24,48,72 in treatment group was lower than that in concurrent control group(P<0.05);PCIA at postoperative hours 12,24,48 in treatment group was lower than that in concurrent control group(P<0.05);AR rate in treatment group at postoperative hours 12,24,48,72 were decreased,and hip-flexion ROM,hip joint ROM in treatment group at postoperative day 7 were more than those in concurrent control group(all P<0.05).Conclusion Hongyuan capsule with multimodal analgesia has good effect,can decrease opioid dosage and reduce AR,and promote the function recovery of hip.
