熄风定颤汤联合多巴丝肼治疗肝肾阴虚型帕金森病的临床疗效及对脑神经递质的影响

摘　　要：目的观察熄风定颤汤联合多巴丝肼治疗肝肾阴虚型帕金森病的临床疗效及对脑神经递质的影响。方法将85例肝肾阴虚型帕金森病患者按照随机数字表法分为2组,对照组42例予多巴丝肼片治疗,治疗组43例在对照组治疗基础上加熄风定颤汤治疗,均治疗3个月。比较2组治疗前后中医证候评分、帕金森病综合评估量表(UPDRS)评分、脑神经递质[S1谱系γ-氨基丁酸(GABA)、S4谱系5-羟色胺(5-HT)、S7谱系去甲肾上腺素(NE)、S11谱系多巴胺(DA)]变化及临床疗效和中医证候疗效。结果治疗后,2组中医证候各项评分及总分均低于本组治疗前(P<0.05),且治疗组震颤、肢体拘痉、运动迟缓、失眠多梦、腰膝痠软评分及总分均低于对照组(P<0.05)。治疗后,2组UPDRS精神行为和情绪、日常活动、运动功能评分及总分均低于本组治疗前(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后,2组GABA均低于本组治疗前(P<0.05),5-HT、NE、DA均高于本组治疗前(P<0.05),且治疗组GABA低于对照组(P<0.05),5-HT、NE、DA均高于对照组(P<0.05)。治疗组临床有效率为86.05%(37/43),中医证候有效率为88.37%(38/43),对照组分别为66.67%(28/42)、69.05%(29/42),治疗组均高于对照组(P<0.05)。结论熄风定颤汤联合多巴丝肼治疗有助于改善肝肾阴虚型帕金森病患者临床症状,提高治疗效果,可能与调节脑内神经递质水平有关。Objective To observe the clinical efficacy of Xifeng Dingchan Decoction combined with madopar for Parkinson's disease(PD)of liver-kidney yin deficiency and its effect on brain neurotransmitter.Methods Totally 85 patients with PD(liver-kidney yin deficiency)were randomly divided into two group,43 patients in treatment group were given Xifeng Dingchan Decoction combined with madopar,and 42 Patients in control group given madopar alone.3-month treatment were performed in order to compare traditional Chinese medicine(TCM)syndrome score,Unified Parkinson's Disease Rating Scale(UPDRS),brain neurotransmitter inclding S1(Gamma-aminobutyric acid[GABA]),S4(5-hydroxytryptamine[5-HT]),S7(norepinephrine[NE]),S11(dopamine[DA]),TCM syndrome efficacy,clinical efficacy between groups.Results After treatment,TCM syndrome scores in groups were decreased than those before treatment(P<0.05);TCM syndrome(trembling,limb spasm,movement retardation,insomnia and dreaminess,soreness and weakness of waist and knees)score were lower in treatment group than in control group(P<0.05);UPDRS scores including mental behavior and emotions,daily activities,motor function in groups were decreased(P<0.05),treatment group was decreased than control group(P<0.05);GABA in groups were decreased(P<0.05),5-HT,NE,DA in groups were increased(P<0.05),and GABA were lower in treatment group than in control group(P<0.05),5-HT,NE,DA were higher in treatment group than in control group(P<0.05).The effective rates of clinical and TCM syndrome in treatment group were better than those in control group(86.05%[37/43]vs 66.67%[28/42],88.37%[38/43]vs 69.05%[29/42],[P<0.05],respectively).Conclusion For patients with PD(liver-kidney yin deficiency),Xifeng Dingchan Decoction combined with madopar is helpful to improve clinical symptoms and treatment effect,which may be associated with regulated brain neurotransmitter levels.
