武汉某医院国家“4+7”试点扩围抗高血压药品使用情况分析

摘　　要：目的为抗高血压药品国家组织药品集中采购(简称集采)政策的制订提供参考。方法收集国家“4+7”试点扩围中选抗高血压药品及与之对应的在用同通用名抗高血压药品在武汉某医院实行“4+7”试点扩围前后1年的相关用药数据,采用药物经济学方法对药品价格、用药数量、用药金额、用药频度(DDDs)和限定日费用(DDC)等数据进行回顾性分析。结果“4+7”试点扩围后,中选抗高血压药品的价格平均降幅达61.94%,最高降幅达80.00%;用量平均增长51.49%;用药金额增长39.49%。同通用名非中选药品的用量平均下降17.83%;用药金额平均下降8.49%,且整体呈下降趋势。实际节省药品费用150.74万元,节省医保资金108.29万元。结论“4+7”试点扩围作为集采制度改革的重要举措,切实减轻了患者的药品费用负担,节省了医保资金,可以不断推进,并不断扩大集采范围。Objective To provide a reference for the formulation of national centralized drug procurement(NCDP)policy of antihypertensive drugs.Methods The relevant medication data of antihypertensive drugs selected in the national″4+7″pilot expansion and the corresponding antihypertensive drugs with the same general name in use in one year before and after the implementation of the″4+7″pilot expansion in a hospital in Wuhan was collected.The data of drug price,the quantity of drug use,the amount of drug use,the defined daily doses(DDDs)and the defined daily cost(DDC)was analyzed retrospectively by the pharmacoeconomics method.Results After the implementation of″4+7″pilot expansion,the average decline of the price of the selected antihypertensive drugs was 61.94%with a maximum falling rate of 80.00%,the quantity of drug use increased by 51.49%on average,and the amount of drug use increased by 39.49%.The quantity of non-selected antihypertensive drugs with the same general name decreased by 17.83%on average,the amount of drug use increased by 8.49%on average and the overall trend showed a downward trend.The actual cost of drugs was saved by 1.5074 million CNY,and the medical insurance fund was saved by 1.0829 million CNY.Conclusion As an important measure of the reform of NCDP system,the″4+7″pilot expansion has effectively reduced the drug cost burden of patients and saved medical insurance fund,which can be continuously promoted,and the range of NCDP can be continusously expanded.
