厚朴排气合剂联合腹部推拿按摩对胸腰椎术后患者胃肠道功能及炎性因子水平的影响

摘　　要：目的探讨厚朴排气合剂联合腹部推拿按摩对胸腰椎术后患者胃肠道功能及炎性因子水平的影响。方法选取医院2019年2月至2020年12月收治的胸腰椎术后患者103例,随机分为观察组(52例)和对照组(51例)。两组患者均予腹部推拿按摩治疗,观察组加用厚朴排气合剂。结果观察组总有效率为90.38%,显著高于对照组的74.51%(P<0.05);观察组患者治疗后肠鸣音恢复、首次排气、首次排便、腹胀消失的时间均显著短于对照组(P<0.05);观察组患者治疗后胃泌素、生长抑素、胃动素的水平均显著高于对照组(P<0.05);观察组患者治疗后肿瘤坏死因子-α、D-乳酸、二胺氧化酶的水平均显著低于对照组(P<0.05);观察组和对照组治疗期间不良反应发生率相当(15.38%比9.80%,P>0.05)。结论厚朴排气合剂联合腹部推拿按摩治疗胸腰椎术后患者的疗效较好,能促进患者术后的胃肠道功能恢复,改善腹胀症状,提高胃肠道激素水平,降低炎性因子水平,且治疗安全性良好。Objective To investigate the effect of Houpu Exhaust Mixture combined with abdominal massage on gastrointestinal function and inflammatory factor level in patients after thoracolumbar surgery.Methods A total of 103 patients after thoracolumbar surgery admitted to the hospital from February 2019 to December 2020 were selected and randomly divided into the observation group(52 cases)and the control group(51 cases).Both groups were treated with abdominal massage,on this basis,the observation group was treated with Houpu Exhaust Mixture.Results The total effective rate in the observation group was 90.38%,which was significantly higher than 74.51%in the control group(P<0.05).After treatment,the time of bowel sound recovery,the first exhaust,the first defecation and the disappearance of abdominal distension in the observation group was significantly shorter than those in the control group(P<0.05),the levels of gastrin(GAS),somatostatin(SST)and motilin(MOT)in the observation group were significantly higher than those in the control group(P<0.05),the levels of tumor necrosis factor-α(TNF-α),D-lactic acid(D-LA)and diamine oxidase(DAO)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group during treatment(15.38%vs.9.80%,P>0.05).Conclusion Houpu Exhaust Mixture combined with abdominal massage is effective and safe in the treatment of patients after thoracolumbar surgery,which can promote the recovery of gastrointestinal function after surgery,improve the symptoms of abdominal distension and the level of gastrointestinal hormones,reduce the level of inflammatory factors.
