摘 要:目的:探讨正常体质量代谢异常(MUNW)和代谢正常超重/肥胖(MHO)两种特殊类型人群的分布以及与糖尿病患病风险的关联。方法:本研究是关于安徽省合肥市1152例30~80岁人群的横断面研究,按统一调查表进行问卷调查,测量身高、体质量、腰围、臀围、血压等。调查对象空腹抽取静脉血,测定血脂、血糖,并接受口服75g葡萄糖耐量试验。将人群分为4组:正常体质量代谢正常(MHNW)组(作为对照组),正常体质量代谢异常(MUNW)组,代谢正常超重/肥胖(MHO)组和代谢异常超重/肥胖组(MUO)。结果:MUNW组和MHO组占男性人群的百分比分别为:7.6%和22.7%,占女性人群的百分比分别为:5.7%和25.9%。纠正年龄、吸烟、糖尿病家族史后运用logistic回归分析发现:不论男女,MUNW组和MUO组糖尿病的患病风险均明显大于对照组(P<0.001),而MHO组患糖尿病的风险与对照组比较没有明显差异(P>0.05)。结论:人群中存在MUNW和MHO两种比较特殊类型的人群。MUNW能显著增加人群糖尿病的患病风险,而MHO则是一种良性肥胖,并不增加人群患糖尿病的风险,鉴别两种特殊类型的人群对于实际的临床治疗工作中有重要的现实指导意义。Objective:The purpose of this study was to estimate the prevalence of metabolically unhealthy normal weight(MUNW)and metabolically healthy overweight/obesity(MHO)phenotypes in Chinese community,and to evaluate the association of the two phenotypes of obesity with risk of diabetes.Method:This was a cross-sectional study of 1152 people aged 30-80 years from the community in Hefei.All the subjects were divided into four groups:metabolically healthy normal weight(MHNW),metabolically unhealthy normal weight(MUNW),metabolically healthy overweight/obesity(MHO),and metabolically unhealthy overweight/obesity(MUO).According to whether there is metabolic syndrome,obesity or overweight,a questionnaire survey shall be conducted according to the unified questionnaire,and the height,weight,waist circumference,hip circumference,blood pressure,etc.shall be measured.The subjects were taken venous blood on an empty stomach,measured blood lipid and blood glucose,and received oral 75g glucose tolerance test.Result:The proportion of MUNW and MHO phenotypes were 7.6%and 22.7%,respectively in men,5.7%and 25.9%respectively in women.After adjusting for age,smoking,and family history of diabetes,MUNW and MUO phenotypes had higher risk of diabetes compared with MHNW phenotype(P<0.001).However,No difference can be found between MHO and MHNW(P>0.05).Conclusion:MUNW and MHO phenotypes were reasonably common in Chinese community.MUNW phenotype can significantly increase the risk of diabetes.However,MHO phenotype was a kind of benigh obesity,and did not increase the risk of diabetes.To distinguish the two special subtypes of obesity was very important for our clinical work.
