摘　　要：目的研究锁定钢板结合钛缆对髋关节置换术后假体周围骨折患者Harris评分与骨折愈合时间的影响。方法利用随机数字表法将我院2018年3月至2020年8月收治的80例髋关节置换术后假体周围骨折患者分为对照组和研究组,各40例。对照组予以锁定钢板治疗,研究组予以锁定钢板结合钛缆治疗。比较两组的治疗效果。结果术后3、6个月,两组的Harris评分均较术前升高,且研究组高于对照组(P<0.05)。研究组的术中失血量明显少于对照组,骨折愈合时间、下床活动时间明显短于对照组(P<0.05)。术后,两组的血浆黏度、FIB水平及红细胞聚集指数均较术前显著升高,但研究组低于对照组(P<0.05)。结论锁定钢板结合钛缆治疗髋关节置换术后假体周围骨折的效果良好,可显著提高Harris评分,减少术中失血量,缩短下床活动时间及骨折愈合时间,减轻对患者血流动力学指标的影响。Objective To study the effects of locking plate combined with titanium cable on Harris score and fracture healing time of patients with periprosthetic fractures after hip replacement.Methods Eighty patients with periprosthetic fractures after hip replacement treated in our hospital from March 2018 to August 2020 were divided into control group and study group according to random number table method,with 40 cases in each group.The control group was treated with locking plate,and the study group was treated with locking plate combined with titanium cable.The therapeutic effects of the two groups were compared.Results At 3 and 6 months after operation,the Harris score of the two groups was higher than that before operation,and that of the study group was higher than the control group(P<0.05).The intraoperative blood loss in the study group was significantly less than that in the control group,and the fracture healing time and out-of-bed activity time in the study group were significantly shorter than those in the control group(P<0.05).After operation,the plasma viscosity,FIB level and erythrocyte aggregation index of the two groups were significantly higher than those before operation,but those of the study group were lower than the control group(P<0.05).Conclusion Locking plate combined with titanium cable in the treatment of periprosthetic fractures after hip replacement has a good effect.It can significantly improve Harris score,decrease intraoperative blood loss,shorten out-of-bed activity time and fracture healing time,and reduce the impact on hemodynamic indexes of patients.
