ELISA、GICT及CLIA检测乙肝病毒标志物的效果比较

摘　　要：目的比较酶联免疫法(ELISA)、免疫胶体金技术(GICT)及电化学发光免疫分析法(CLIA)检测乙肝病毒标志物的效果。方法选取2018年7月至2020年7月我院收治的200例疑似乙肝患者作为研究对象,分析ELISA、GICT及CLIA对乙肝病毒标志物的检测阳性率。以病理诊断为金标准,比较ELISA、GICT及CLIA对HBsAg的诊断效能。结果CLIA对HBcAb、HBeAb、HBeAg、HBsAb、HBsAg的检测阳性率最高,其次为ELISA、GICT。以病理诊断为金标准,ELISA、GICT及CLIA对HBsAg的诊断灵敏度分别为79.41%、63.73%、97.06%,特异度分别为86.73%、80.61%、96.94%,准确度分别为83.00%、72.00%、97.00%。CLIA对HBsAg的诊断灵敏度、特异度、准确度高于ELISA、GICT,差异具有统计学意义(P<0.05)。结论CLIA对乙肝病毒标志物的检测阳性率高于ELISA、GICT,CLIA对HBsAg的诊断灵敏度、特异度、准确度高于ELISA、GICT。Objective To compare the effects of enzyme linked immunosorbent assay(ELISA),immune colloidal gold technique(GICT)and chemiluminescence immunoassay(CLIA)in the detection of hepatitis B virus markers.Methods A total of 200 patients with suspected hepatitis B admitted in our hospital from July 2018 to July 2020 were selected as the research objects.The detection positive rates of ELISA,GICT and CLIA for hepatitis B virus markers were analyzed.Taking pathological diagnosis as the gold standard,the diagnostic efficacy of ELISA,GICT and CLIA for HBsAg was compared.Results The detection positive rates of CLIA for HBcAb,HBeAb,HBeAg,HBsAb and HBsAg were the highest,followed by ELISA and GICT.Taking pathological diagnosis as the gold standard,the diagnostic sensitivity of ELISA,GICT and CLIA for HBsAg was 79.41%,63.73%and 97.06%,respectively,the specificity was 86.73%,80.61%and 96.94%,respectively,and the accuracy was 83.00%,72.00%and 97.00%,respectively.The diagnostic sensitivity,specificity and accuracy of CLIA for HBsAg were higher than those of ELISA and GICT,and the differences were statistically significant(P<0.05).Conclusion The positive rate of CLIA in the detection of hepatitis B virus markers is higher than that of ELISA and GICT.The diagnostic sensitivity,specificity and accuracy of CLIA for HBsAg are higher than those of ELISA and GICT.
