温针灸结合艾灸神阙穴治疗寒凝血瘀型原发性痛经的效果及对痛经症状、前列腺素、β-EP水平的影响

摘　　要：目的探讨温针灸结合艾灸神阙穴治疗寒凝血瘀型原发性痛经的效果及对痛经症状、前列腺素、β-EP水平的影响。方法选择2017年5月至2020年6月收治的94例寒凝血瘀型原发性痛经患者为研究对象,采用随机数字表法将其分为对照组与观察组,各47例。对照组接受常规药物治疗,观察组在对照组基础上加用温针灸结合艾灸神阙穴治疗。比较两组的临床疗效、中医证候积分、子宫动脉血液动力学指标、痛经症状的严重程度与维持时间、疼痛情况、前列腺素及β-EP水平。结果观察组的治疗总有效率为95.74%,明显高于对照组的80.85%(P<0.05)。治疗后,两组患者的主症、次症、舌脉象评分均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组患者的RI、PI及A/B均较治疗前减小,且观察组小于对照组(P<0.05)。治疗后,两组患者的CMSS、VAS评分均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组患者的PGE_(2)、β-EP水平高于治疗前,PGF_(2α)水平低于治疗前,且观察组优于对照组(P<0.05)。结论温针灸结合艾灸神阙穴治疗寒凝血瘀型原发性痛经的效果理想,可有效减轻疼痛程度及痛经症状,改善子宫动脉血液动力学指标,调节前列腺素与β-EP水平。Objective To explore the effect of warm acupuncture combined with moxibustion at Shenque point on primary dysmenorrhea of cold coagulation and blood stasis type and its influences on dysmenorrhea symptoms,prostaglandin andβ-EP levels.Methods Ninety-four patients with primary dysmenorrhea of cold coagulation and blood stasis type treated from May 2017 to June 2020 were selected as the research objects and divided into control group and observation group according to random number table method,with 47 cases in each group.The control group received routine drug treatment,and the observation group was treated with warm acupuncture combined with moxibustion at Shenque point on the basis of the control group.The clinical efficacy,TCM syndromes scores,hemodynamic indexes of uterine artery,severity and maintenance time of dysmenorrhea symptoms,pain,prostaglandin andβ-EP levels were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.74%,which was significantly higher than 80.85%in the control group(P<0.05).After treatment,the scores of main symptoms,secondary symptoms,tongue and pulse of the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).After treatment,the RI,PI and A/B of the two groups were smaller than those before treatment,and those of the observation group were smaller than the control group(P<0.05).After treatment,the CMSS and VAS scores of the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).After treatment,the levels of PGE2 andβ-EP in the two groups were higher than those before treatment,the level of PGF2αwas lower than that before treatment,and those of the observation group were better than the control group(P<0.05).Conclusion Warm acupuncture combined with moxibustion at Shenque point in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type has a ideal effect.It ca
