鼻内镜下钬激光治疗放射性后鼻孔闭锁的临床分析

摘　　要：目的探讨鼻内镜下钬激光治疗鼻咽癌放疗后后鼻孔闭锁的原理和疗效。方法回顾性分析24例放射性后鼻孔闭锁的患者临床资料,均采用鼻内镜下钬激光辅助经鼻腔切除膜性闭锁,术后分期观察疗效。结果随访3年,22例患者鼻腔达到正常通气,2例复发狭窄再次钬激光并放置硅胶管6月达正常通气。结论鼻内镜下钬激光治疗鼻咽癌放疗后后鼻孔闭锁,具有操作简便、术野清晰、组织损伤小、患者痛苦少、远期疗效可靠等优点,是治疗放射性后鼻孔闭锁较为理想和有效的方法。Objective To investigate the principle and effect of nasal endoscopic holmium laser in the treatment of choanal atresia after radiotherapy for nasopharyngeal carcinoma.Methods The clinical data of 24 patients with choanal atresia after radiotherapy who were treated with endonasal resection of membranous atresia assisted by nasal endoscopic holmium laser were retrospectively reviewed.The postoperative curative effect was observed by stages.Results With a 3-year follow-up,22 patients achieved normal nasal ventilation,while other 2 patients suffered from recurrent stenosis and achieved normal nasal ventilation after receiving holmium laser once again and accepting placement of silicone tube for 6 months.Conclusions Nasal endoscopic holmium laser in treating choanal atresia after radiotherapy for nasopharyngeal carcinoma had the advantages of simple operation,clear operation field,less tissue damage,less pain,reliable long-term efficacy and so on.It was an ideal and effective method for the treatment of choanal atresia after radiotherapy.
