摘 要:目的基于三维对比增强磁共振静脉造影(3D-CE-MRV)技术探讨成人乳突导静脉(MEV)的解剖学特征.方法分析124例受检者的头颅3D-CE-MRV资料,经多平面重建后评估MEV的发生情况、走行、起始位置及内口连接处直径.结果124例中出现双侧MEV占53.2%,单侧MEV占37.9%,无MEV占8.9%;右侧MEV出现率为73.4%,左侧为71%,两侧别间出现率差异无统计学意义(χ2=0.181,P=0.671);MEV在骨内走行形态大致分为直线型(50.2%)、弯曲型(42.3%)、曲折型(7.5%);MEV起始于乙状窦后外壁上段占24.9%、中段占5.9%、下段占69.2%;右侧MEV内口连接处直径为(2.02±0.65)mm,左侧为(1.87±0.66)mm,两侧别间直径差异无统计学意义(t=1.473,P=0.123).结论MEV个体异质性较大,3D-CE-MRV技术可清晰显示其解剖学特征.术前强调应用3D-CE-MRV检查来全面了解、评估MEV的情况,将对外科手术具有重要理论意义和临床指导价值.Objective To explore the anatomic characteristics of the mastoid emissary vein(MEV)of adults based on three-dimensional contrast-enhanced magnetic resonance venography(3D-CE-MRV).Methods The head 3D-CE-MRV data of 124 patients were analyzed,the occurrence,intra-osseous shape,starting position and diameter of internal connection of the MEV were evaluated after mult-i planar reconstruction.Results Among the 124 patients,53.2%of them had bilateral MEV,37.9%had unilateral MEV and 8.9%didn’t have MEV.The occurrence of right MEV was 73.4%,while that of the left MEV was 71%,and there was no statistically significant difference between the two sides in the occurrence of MEV(χ2=0.181,P=0.671).The intra-osseous shape of the MEV was roughly divided into straight type(50.2%),curved type(42.3%)and tortuous type(7.5%).The MEV originated from the sigmoid sinus along its posterolateral wall,of which the upper third accounted for 24.9%,the middle third for 5.9%,the lower third for 69.2%.The diameter of internal connection of the right MEV was(2.02±0.65)mm,while that of the left was(1.87±0.66)mm,and thus there was no statistically significant difference in diameter between the two sides(t=1.473,P=0.123).Conclusion There are complicated anatomical variations in the MEV,which can be clearly displayed by 3D-CE-MRV.The application of 3D-CE-MRV to fully understand and evaluate the MEV will be of important theoretical significance and clinical guiding value for surgery.
