摘　　要：目的探讨IDEAL-IQ序列在绝经后女性骨质疏松症的应用诊断价值.方法选取绝经后女性骨质疏松症患者24例、骨量减少患者14例以及健康志愿者17例,采用IDEAL-IQ序列和双能X线吸收法(DXA)测量椎体的脂肪分数(FF)、R2∗和骨密度(BMD).应用Pearson相关分析,比较FF、R2∗值与BMD的相关性,利用受试者工作特征(ROC)曲线评价单独使用FF、R2∗以及将FF和R2∗联合应用对骨质疏松症的诊断效能.结果FF值与BMD呈中度负相关,相关系数(r)=-0.648(P<0.001),R2∗值与BMD呈中度正相关,r=0.593(P<0.001).FF、R2∗以及将FF和R2∗联合应用诊断骨质疏松症的曲线下面积(AUC)分别为0.837、0.814、0.870.结论IDEAL-IQ序列能够定量评价骨髓脂肪含量,并在一定程度反映骨小梁结构变化,为绝经后女性骨质疏松症的诊断提供重要价值.Objective To investigate the diagnostic value of IDEAL-IQ sequence in postmenopausal women with osteoporosis.Methods 24 patients with osteoporosis,14 patients with osteopenia and 17 healthy volunteers were enrolled,who were all postmenopausal women.The vertebral fat fraction(FF),R2∗and bone mineral density(BMD)were measured by IDEAL-IQ sequence and dual energy X-ray absorptiometry(DXA).Pearson correlation analysis was used to compare the correlation between FF and R2∗values and BMD,and receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficacy of FF or R2∗and their combination for osteoporosis.Results FF values was moderately negatively correlated with BMD,with a correlation coefficient(r)=-0.648(P<0.001),and R2∗value was moderately positively correlated with BMD,with r=0.593(P<0.001).The area under the curve(AUC)of FF,R2∗and combined diagnosis of osteoporosis were 0.837,0.814 and 0.870,respectively.Conclusion The IDEAL-IQ sequence can quantitatively evaluate the fat content of bone marrow and reflect the changes of trabecular structure to a certain extent,which provides important value for the diagnosis of osteoporosis in postmenopausal women.
