误诊为表皮样囊肿的毛囊瘤一例

摘　　要：20岁男性患者,项部皮肤肿物6个月余。6个月前,患者项部皮肤出现一约绿豆大小的丘疹,缓慢增大,并出现红肿、疼痛,挤压后有皮脂样物质流出,考虑表皮样囊肿。行切开引流治疗,术后在创面愈合瘢痕处穿出一簇白色毳毛。手术完整切除,组织病理示:真皮内可见异常扩张的毛囊,开口于表皮,周围为放射状排列的次级毛囊。诊断:毛囊瘤。术后随访6个月无复发。A 20-year-old male patient presented with a papule on the posterior neck for 6 months.Six months ago,one mung bean-sized papule presented on the posterior cervical skin of the patient and increased slowly.The lesions became swollen and painful,and ivorywhite sebaceous material could be easily discharged under slight pressure from the lesions.So epidermoid cyst was diagnosed,and treated by incision drainage.Later,a tuft of white vellus hair grew on the scar.The patient was treated with surgical resection at the author’s hospital,and the histopathologic examination showed a dilated hair follicle opening in the epidermis,surrounded by radiated arranged numerous differentiated secondary vellus follicles.The diagnosis of the patient was trichofolliculoma.There was no recurrence after a follow up of 6 months.
