口腔固定修复中排龈效果影响因素研究进展

摘　　要：口腔固定修复因为其保存患牙,修复效率高,使用方便,恢复咀嚼功能高,美观性良好等优点,为患者较容易接受。评估固定修复体成功与否,除美观及功能方面外,密合也是固定修复体的灵魂。决定其密合程度,跟能否清晰准确复制口内预备体边缘情况有直接关系。模型精准清晰再现龈下肩台与印模上一致的软硬组织界限可确保修复成功。排龈是修复过程中的关键,良好的排龈效果可提供完整肩台以及精准印模。排龈是使用不同种类排龈排料分离游离龈缘和牙体组织,将龈下区域暴露出来,从而进行取模或口扫。临床研究发现,排龈效果的影响因素较多,包括患者自身牙龈状态、操作医师的评价标准以及方法、不同排龈方法以及排龈使用的材料、排龈使用时间等。根据影响因素进行针对性措施处理,可提升排龈效果,促进口腔牙体固定修复。本文综述了口腔固定修复过程中排龈效果影响因素的研究进展,为制订针对性的措施提供帮助。Oral fixation prosthesis is easy to be accepted by patients because of its advantages of preserving affected teeth,high repair efficiency,convenient use,high restoration of chewing function,good aesthetics and so on.In addition to aesthetic and functional aspects,sealing is also the soul of the fixed prosthesis in evaluating the success of the fixed prosthesis.The determination of its tightness is directly related to whether it can clearly and accurately copy the edge of the preparation in the mouth.The model accurately and clearly reproduces the consistent soft and hard tissue boundary between the subgingival shoulder and the dental impression,which can ensure the success of the repair.Gingival retraction is the key in the repair process.Good gingival retraction effect can provide complete shoulder and accurate impression.Gingival retraction is to use different kinds of gingival retraction materials to separate the free gingival margin and tooth tissue,expose the subgingival area,and then take the mold or oral scan.Clinical studies have found that there are many influencing factors of gingival retraction effect,including patients’ own gingival state,evaluation criteria and methods of operators,different gingival retraction methods,materials used for gingival retraction,gingival retraction time and so on.Targeted measures base on to the influencing factors can improve the effect of gingival retraction and promote the fixation and repair of oral teeth.This paper reviews the research progress on the influencing factors of gingival retraction effect in the process of oral fixation and repair,so as to provide help for formulating targeted measures.
