摘 要:分析河北国际旅行卫生保健中心出入境体检中190例疑似肺结核患者和45例健康人,使用结核抗体检测和T-SPOT检测的诊断结果。统计并评价结核抗体和T-SPOT独立检测以及二者联合检测的漏诊率、特异性和阳性率。2种方法独立检测,能够有效区分疑似结核患者(P<0.05)。疑似结核患者中,T-SPOT方法的阳性率和特异性均高于结核抗体方法,且结核抗体方法的漏诊率高于T-SPOT方法(P<0.05)。相比结核抗体方法和T-SPOT方法独立检测,联合检测的阳性率为89.47%、特异性为87.50%,均显著提高(P<0.05);漏诊率为7.14%,显著降低(P<0.05)。相较于结核抗体方法,T-SPOT方法检出率和特异性更高。结核抗体方法与T-SPOT方法联合检测,相较于单一检测效果更加显著,在进出境肺结核筛查中具有重要临床价值。The diagnostic results of 190 suspected tuberculosis cases and 45 healthy persons by TB antibody and T-SPOT detection were analyzed retrospectively.The rates of missed diagnosis,specificity and positive rate of TB antibody and T-SPOT independent and combined detection were evaluated.The two methods were independently detected and could effectively distinguish suspected tuberculosis patients(P<0.05).In suspected tuberculosis cases,the positive rate and specificity of T-SPOT were higher than that of TB antibody,and the rate of missed diagnosis of TB antibody was higher than that of T-SPOT(P<0.05).Compared with the two methods independent detection,the positive rate and specificity of the combined method were 89.47%and 87.50%respectively,which were significantly higher than those of the antibody method and T-SPOT method(P<0.05).The rate of missed diagnosis was 7.14%,significantly decreased(P<0.05).The detection rate and specificity of T-SPOT were more higher than that of TB antibody detection.The combined detection of TB antibody and T-SPOT was more effective than single detection.It had important clinical value in the screening of entry and exit pulmonary tuberculosis.
