前列腺尿道角对前列腺增生患者经尿道前列腺电切术后疗效的指导价值研究

摘　　要：目的:探讨前列腺尿道角(PUA)对前列腺增生患者经尿道前列腺电切术(TURP)术后疗效的指导价值。方法:回顾性分析成都市双流区中医医院2020年1月-2020年10月收治的48例行TURP手术患者的临床资料。术前对所有患者经直肠超声(TRUS)检查测定前列腺体积(PV)及前列腺尿道角(PUA),根据测得PUA值(40.57±8.36),以40°为界限分为PUA<40°组、PUA≥40°组。术前及术后3个月对两组患者行国际前列腺症状评分(IPSS)、最大尿流率(Qmax)资料检测,并以手术前后IPSS评分之差(术后-术前:△IPSS)、手术前后Qmax之差(术后-术前:△Qmax)作为评估术后疗效指标,利用Pearson直线相关性分析评估PUA与△IPSS、△Qmax之间的相关性。结果:PUA≥40°组的PV(48.78±6.68)明显大于PUA<40°组(40.24±8.56)(P<0.05);两组术后与术前IPSS、Qmax比较差异均有统计学意义(P<0.05);Pearson直线相关性分析显示PUA与△Qmax存在正相关(r=0.35,P<0.05),PUA与△IPSS存在负相关(r=-0.29,P<0.05)。结论:PUA与TURP术后疗效存在相关性,TURP手术能通过缩小BPH患者的PUA来降低IPSS评分,增加Qmax,减轻LUTS症状;BPH患者术前PUA越大,TURP术后疗效可能更佳。Objective:To investigate the guiding value of prostate urethral angle(PUA)on the postoperative efficacy of transurethral resection of prostate(TURP).Methods:A retrospective analysis was made on 48 patients who underwent TURP in Shuangliu District Hospital of Traditional Chinese Medicine from January 2020 to October 2020.Transrectal ultrasound(TRUS)was performed in all patients before operation to determine prostate volume(PV)and prostate urethral angle(PUA).According to the measured PUA value(40.57±8.36),the patients were divided into PUA<40°group and PUA≥40°group with 40°as the boundary.The international prostate symptom score(IPSS)and maximum urinary flow rate(Qmax)data of the two groups were detected before and 3 months after operation.The difference between IPSS scores before and after operation(postoperative-preoperative:△IPSS)and the difference between Qmax before and after operation(postoperative-preoperative:△Qmax)were used as the evaluation indexes of postoperative curative effect.Pearson linear correlation analysis was used to evaluate the correlation between PUA and△IPSS and△Qmax.Results:PV of PUA≥40°group was(48.78±6.68),which was significantly higher than that(40.24±8.56)of PUA<40°group(P<0.05).The differences in IPSS and Qmax between the two groups were statistically significant(P<0.05).Pearson linear correlation analysis showed that PUA was positively correlated with△Qmax(r=0.35,P<0.05),and PUA was negatively correlated with△IPSS(r=-0.29,P<0.05).Conclusion:PUA is correlated with the curative effect of TURP.TURP can reduce IPSS score,increase Qmax and relieve LUTS symptoms by reducing PUA in BPH patients.The greater the preoperative PUA of BPH patients,the better the efficacy after TURP.
